青少年急性白血病诱导化疗期患者创伤后成长水平及其对生存质量的影响

摘　　要：目的探讨青少年急性白血病诱导化疗期患者创伤后成长水平及其对生存质量的影响。方法研究对象为2020年6-12月期间新疆医科大学第一附属医院血液科收治的192名青少年急性白血病诱导化疗期患者,对其采用一般资料调查问卷、创伤后成长评估量表和白血病生存质量评估量表进行问卷调查。结果青少年急性白血病诱导化疗期患者创伤后成长和生存质量总分分别为(60.72±12.01)分和(117.16±22.13)分,二者总分及各维度之间均呈正相关(P<0.05);单因素分析结果显示,不同年龄、家庭人均月收入、家庭类型、已接受化疗次数、近一周内是否出现出血的青少年急性白血病诱导化疗期患者创伤后成长得分不同(P<0.05);分层回归分析结果显示青少年急性白血病诱导化疗期患者生存质量可解释创伤后成长得分42.3%的变异。结论创伤后成长对青少年急性白血病诱导化疗期患者生存质量有一定影响,是能够帮助患者增加疾病自我管理能力的促进力量,医护人员在关注患者负性情绪的同时,可通过制定心理调试策略帮助青少年急性白血病诱导化疗期患者进行积极应对,提高其生存质量。Objective To explore the post-traumatic growth level and its effect on quality of life in adolescent patients with acute leukemia during induction chemotherapy.Methods The subjects of this study were 192 adolescent patients with acute leukemia during induction chemotherapy in the Hematology Department of the First Affiliated Hospital of Xinjiang Medical University from June 2020 to December 2020.The subjects were surveyed by general data questionnaire,post-traumatic growth assessment scale and leukemia quality of life assessment scale.Results The total score of post-traumatic growth level and quality of life in the adolescent patients with acute leukemia during primary treatment of induction chemotherapy were 60.72±12.01 and 117.16±22.13 respectively,and the total scores and dimensions were positively correlated(P<0.05).Univariate analysis showed that the post-traumatic growth scores of thethem were different with different ages,family per capita monthly income,family type,times of receiving chemotherapy,and whether bleeding occurred within the last week(P<0.05).Stratified regression analysis showed that quality of life could explain 42.3%variation of post-traumatic growth score of them.Conclusion Post-traumatic growth of adolescent patients with acute leukemia during induction chemotherapy have effects on quality of life,is to be able to help patients increase the impetus for self-management ability on disease,when medical staff pay more attention to the negative emotions,they also develop psychological debugging strategy to cope with the positive response of the adolescents patients with acute leukemia during induction chemotherapy,to improve their quality of life.
