喉癌患者支撑喉镜下低温等离子切除术中声门暴露困难的影响因素

摘　　要：目的探讨喉癌患者支撑喉镜下低温等离子切除术中声门暴露困难的影响因素。方法对178例喉癌患者行支撑喉镜下低温等离子切除术,按照声门暴露状况分为暴露困难组37例与暴露正常组141例。比较两组性别、年龄、体质量指数、Yamamoto分级、改良Mallampati指数分级、颈周长、张口度、正中位舌颏距离、甲颏距离、胸颏距离、水平甲颏距离、垂直甲颏距离。结果暴露困难组体质量指数≥25.0 kg·m^(2)、Yamamoto分级3级-4级、改良Mallampati指数分级3级-4级、颈周长>38.0 cm、张口度≤3.0 cm患者占比显著高于暴露正常组(P<0.05)。体质量指数≥25.0 kg·m^(2)、Yamamoto分级3级-4级、改良Mallampati指数分级3级-4级、颈周长>38.0 cm、张口度≤3.0 cm是喉癌患者支撑喉镜下低温等离子切除术中声门暴露困难的独立危险因素(OR>1,P<0.05)。结论体质量指数、Yamamoto分级、改良Mallampati指数分级、颈周长、张口度是喉癌患者支撑喉镜下低温等离子切除术中声门暴露困难的影响因素,术前可通过检测以上指标评估患者的手术风险。Objective To analyze the influencing factors of difficult glottic exposure during low-temperature plasma resection under suspension laryngoscope in laryngeal carcinoma patients.Methods A total of 178 laryngeal carcinoma patients underwent low-temperature plasma resection under suspension laryngoscope,and they were divided into difficult exposure group(37 cases)and normal exposure group(141 cases)according to intraoperative laryngeal exposure status.The gender,age,body mass index(BMI),Yamamoto grading,modified Mallampati index(MMI)grading,neck circumference,mouth opening,hyoid-mental distance(HMD),thyromental distance(TMD),sternomental distance(SMD),horizontal distance between nail and chin(HTMD),vertical distance between nail and chin(VTMD)were compared between the two groups.Results The proportion of BMI greater than or equal to 25.0 kg·m^(2),Yamamoto grading(grade 3-4),MMI grading(grade 3-4),neck circumference greater than 38.0 cm,mouth opening less than or equal to 3.0 cm in difficult exposure group was higher than that in normal exposure group(P<0.05).BMI greater than or equal to 25.0 kg·m^(2),Yamamoto grading(grade 3-4),MMI grading(grade 3-4),neck circumference greater than 38.0 cm,mouth opening less than or equal to 3.0 cm were independent risk factors of difficult glottic exposure in low-temperature plasma resection under suspension laryngoscope(OR>1,P<0.05).Conclusions BMI,Yamamoto grading,MMI grading,neck circumference,mouth opening are the influencing factors of difficult glottic exposure in low-temperature plasma resection under suspension laryngoscope,and the above indexes can be detected to evaluate the surgical risk of patients.
