神经肌肉电刺激联合向心性缠手治疗脑梗死后肩手综合征效果分析

摘　　要：目的探讨神经肌肉电刺激联合向心性缠手治疗脑梗死后肩手综合征的应用效果。方法将200例脑梗死后肩手综合征患者按照随机数字表法分为两组,各100例。两组均给予神经肌肉电刺激治疗,研究组在此基础上给予向心性缠手治疗。干预前后采用护士住院患者观察量表、世界卫生组织生存质量测定量表、阴性症状量表评定两组生活质量、护理满意度及临床症状,采用简式Fugl-Meyer运动功能评分法评定两组运动功能,自拟护理满意度评分表评定护理满意度,比较两组临床症状评分及肿胀减轻程度。结果干预后两组护士住院患者观察量表、世界卫生组织生存质量测定量表、阴性症状量表、简式Fugl-Meyer运动功能评分法评分均显著高于干预前(P<0.01),研究组高于对照组(P<0.01);两组肩痛、关节活动度、浮肿症状评分均显著低于干预前(P<0.01),研究组显著低于对照组(P<0.01);研究组护理满意度评分显著高于对照组(P<0.01),肿胀减轻程度显著优于对照组(P<0.01)。结论神经肌肉电刺激联合向心性缠手治疗脑梗死后肩手综合征效果显著,能显著提高患者的生活质量,促进神经功能恢复,提升患者满意度。Objective To explore the application effect of neuromuscular electrical stimulation combined with centripetal hand entanglement in the treatment of shoulder-hand syndrome after cerebral infarction.Methods A total of 200 patients with shoulder-hand syndrome after cerebral infarction were divided into two groups according to the random number table method,each with 100 cases.Both groups were given neuromuscular electrical stimulation therapy,and the study group was given centripetal hand entanglement therapy on this basis.Before and after the intervention,the nurses observation scale for inpatient evaluation(NOSIE),the World Health Organization Quality of Life assessment(WHOQOL-100),and the scale for assessment of negative symptoms(SANS)were used to assess the quality of life,nursing satisfaction and clinical symptoms of the two groups,and the simplified Fugl-Meyer motor function scale was used to assess the motor function of the two groups,and the self-made nursing satisfaction rating scale was used to evaluate the nursing satisfaction.The clinical symptom scores and the swelling reduction degree were compared between the two groups.Results After the intervention,the NOSIE,WHOQOL-100,SANS,and simplified Fugl-Meyer motor function scale scores of the two groups were significantly higher than those before the intervention(P<0.01),and the study group was higher than the control group(P<0.01).The scores of shoulder pain,joint range of motion,and edema symptoms in the two groups were significantly lower than those before the intervention(P<0.01),the study group was significantly lower than the control group(P<0.01).The nursing satisfaction score of the study group was significantly higher than that of the control group(P<0.01),and the swelling reduction degree was significantly better than that of the control group(P<0.01).Conclusions Neuromuscular electrical stimulation combined with centripetal hand entanglement is effective in treating SHS after cerebral infarction,which can significantly improve the quality of life
