胃癌合并Hp-L感染患者MIF、MMP-9、VEGF和炎症因子的表达水平及临床意义

摘　　要：目的检测胃癌合并幽门螺杆菌L型(Hp-L)感染患者巨噬细胞移动抑制因子(MIF)、基质金属蛋白酶-9(MMP-9)、血管内皮生长因子(VEGF)和炎症因子的表达水平,并探讨其临床意义。方法选择2017年2月至2019年2月西安交通大学第二附属医院收治的172例胃癌患者进行研究,检测胃癌组织、癌旁组织中Hp-L感染阳性率,并比较胃癌Hp-L感染阳性和阴性患者MIF、MMP-9、VEGF表达阳性率、血清白细胞介素(IL)-6和IL-1β表达以及不同Hp-L感染程度患者上述指标的表达情况,采用Pearson法分析Hp-L感染阳性与MIF、MMP-9、VEGF及炎症因子的相关性。结果胃癌组织中Hp-L感染阳性率为69.19%,明显高于癌旁组织的12.21%,差异有统计学意义(P<0.05);胃癌Hp-L感染阳性患者的MIF、MMP-9、VEGF表达阳性率及血清IL-6、IL-1β水平分别为76.47%、82.50%、85.71%、(25.05±3.71)pg/mL、(28.31±3.42)pg/mL,明显高于Hp-L阴性者的13.21%、20.75%、28.30%、(16.23±2.04)pg/mL、(14.07±2.31)pg/mL,差异均有统计学意义(P<0.05);重度Hp-L感染患者的MIF、MMP-9、VEGF表达阳性率及血清IL-6、IL-1β水平分别为94.44%、100.00%、100.00%、(36.85±3.61)pg/mL、(38.40±3.02)pg/mL,中度感染患者分别为78.57%、83.93%、87.50%、(26.73±3.05)pg/mL、(27.93±3.51)pg/mL,轻度感染患者分别为48.15%、55.56%、62.96%、(15.05±2.74)pg/mL、(18.01±2.49)pg/mL,重度感染患者的上述指标明显高于中度和轻度感染者,中度感染患者明显高于轻度感染者,差异均有统计学意义(P<0.05);经Pearson相关性分析结果显示,Hp-L感染阳性与MIF、MMP-9、VEGF、IL-6、IL-1β水平均呈正相关(P<0.05)。结论MIF、MMP-9、VEGF、IL-6、IL-1β水平与胃癌Hp-L感染阳性均呈正相关,且随着感染程度的加重表达越高。Objective To observe the expression of macrophage migration inhibitory factor(MIF),matrix metalloproteinase-9(MMP-9),vascular endothelial growth factor(VEGF),and inflammatory factors in gastric cancer patients with Helicobacter pylori L-form(Hp-L)infection,and to explore its clinical significance.Methods A total of 172 patients with gastric cancer admitted to the Second Affiliated Hospital of Xi'an Jiaotong University from February 2017 to February 2019 were selected for the study.The positive rate of Hp-L infection in gastric cancer tissues and adjacent tissues was detected,and the positive rates of MIF,MMP-9,VEGF,serum interleukin-6 and IL-1βin Hp-L positive and negative gastric cancer patients and different Hp-L infection level were compared.Pearson's correlation analysis was used to analyze the correlation between Hp-L infection positive and MIF,MMP-9,VEGF,and inflammatory factors.Results The positive rate of Hp-L infection in gastric cancer tissues was 69.19%,which was significantly higher than 12.21%in adjacent tissues(P<0.05);the positive rates of MIF,MMP-9,VEGF,and serum IL-6,IL-1βlevels were 76.47%,82.50%,85.71%,(25.05±3.71)pg/mL,(28.31±3.42)pg/mL in the Hp-L positive patients with gastric cancer,which were significantly higher than 13.21%,20.75%,28.30%,(16.23±2.04)pg/mL,(14.07±2.31)pg/mL of Hp-L negative patients(P<0.05);the positive rates of MIF,MMP-9,VEGF,serum IL-6,and IL-1βlevels were 94.44%,100.00%,100.00%,(36.85±3.61)pg/mL,(38.40±3.02)pg/mL in patients with severe Hp-L infection,78.57%,83.93%,87.50%,(26.73±3.05)pg/mL,(27.93±3.51)pg/mL in patients with moderate infection,and 48.15%,55.56%,62.96%,(15.05±2.74)pg/mL,(18.01±2.49)pg/mL in patients with mild infection;the results were significantly higher in patients with severe infection than those in patients with moderate and mild infections,and also in patients with moderate infection than in patients with mild infection,with statistically significant differences(P<0.05).Spearman correlation analysis showed that Hp-L infection positive w
