经圆孔与卵圆孔上颌神经射频介入治疗三叉神经痛的疗效比较

摘　　要：目的探讨经圆孔上颌神经射频介入治疗三叉神经痛的疗效。方法选取2016年1月—2019年3月在笔者医院治疗的原发性三叉神经痛患者150例作为研究对象,根据治疗方案分为观察组(n=74)和对照组(n=76),观察组行经圆孔上颌神经射频介入治疗,对照组行经卵圆孔上颌神经射频介入治疗,观察2组治疗前后疼痛视觉模拟评分(VAS)和不良反应,检测治疗前后5-羟色胺(5-HT)、超氧化物歧化酶(SOD)和β-内啡肽(β-EP)。结果观察组和对照组治疗后1 d、7 d、1个月和3个月时VAS评分比较,差异无统计学意义(P>0.05);观察组治疗后7 d时SOD和β-EP分别为(132.29±22.32)U/ml和(176.29±28.81)ng/L,明显高于对照组(P<0.05);观察组和对照组治疗后7 d时5-HT比较,差异无统计学意义(P>0.05);观察组不良反应发生率为4.05%,明显低于对照组(P<0.05)。结论经圆孔上颌神经射频介入治疗三叉神经痛有较好的效果,其不良反应少,有助于提高患者血清5-HT、SOD和β-EP水平。Objective To explore the effect of radiofrequency interventional treatment of trigeminal neuralgia through the circular hole.Methods 150 patients with primary trigeminal neuralgia treated in our hospital from January 2016 to March 2019 were selected.According to the treatment plan,the patients were divided into the observation group(n=74)and the control group(n=76).Observation group were treated with radio frequency intervention through the circular hole,and the control group were treated with radio frequency intervention through the foramen oval.Visual analogue pain scale(VAS)and adverse reactions were observed before and after treatment,5-hydroxytryptamine(5-HT),superoxide dismutase(SOD)andβ-endorphin(β-EP)were detected before and after treatment.Results There was no significant difference in VAS scores at 1d,7d,1 month and 3 months between the observation group and control group(P>0.05);On the 7th day after treatment,SOD andβ-EP in observation group were(132.29±22.32)U/ml and(176.29±28.81)ng/L,which were significantly higher than those in control group(P<0.05);There was no significant difference in 5-HT at the 7th day after treatment between observation group and control group(P>0.05);The incidence of adverse reactions in observation group was 4.05%,which was significantly lower than that in control group(P<0.05).Conclusion Radiofrequency interventional treatment of trigeminal neuralgia through the circular hole has a good effect and less side effect,which is helpful to improve the levels of 5-HT,SOD andβ-Ep.
