摘  要:目的观察电针深刺对三叉神经痛(TN)大鼠神经节中p-ERK蛋白及Kv3.4、Kv4.3基因表达的影响及作用机制。方法2019年1月—2020年1月在首都医科大学实验中心进行实验。雄性SD大鼠48只,按简单随机数字表法分成假手术组、模型组、电针组,每组16只。模型组、电针组采用眶下神经慢性缩窄环术建立TN模型,电针组建模14 d后接受电针深刺治疗14 d。采用BME-403型细丝法检测仪监测各组大鼠痛阈,免疫组织化学法检测p-ERK、ERK蛋白表达,实时荧光定量PCR(qPCR)检测Kv3.4、Kv4.3 mRNA水平。结果3组大鼠建模前1 d痛阈值比较差异均无统计学意义(P>0.05),造模后7 d,模型组大鼠痛阈高于假手术组(P<0.05),造模后14 d,模型组大鼠痛阈值低于假手术组(P<0.05),而电针组与模型组差异均无统计学意义(P>0.05)。治疗后7 d、14 d电针组痛阈值均高于模型组(P<0.05)。治疗后14 d,模型组p-ERK、ERK免疫荧光强度值显著高于假手术组,电针组显著低于模型组(P<0.05)。治疗后14 d,模型组Kv3.4mRNA、Kv4.3mRNA低于假手术组,电针组高于模型组(P<0.05)。结论电针深刺治疗TN可提高痛阈,其机制与抑制大鼠三叉神经节中p-ERK、ERK蛋白表达及上调Kv3.4、Kv4.3基因表达有关。Objective To observe the effect of deep electroacupuncture on the expression of p-ERK protein and Kv3.4 and Kv4.3 genes in the ganglion of trigeminal neuralgia(TN)rats and the mechanism of action.Methods Forty-eight male SD rats were divided into sham operation group,model group,and electroacupuncture group according to a simple random number table method,with 16 rats in each group.The model group and the electroacupuncture group used chronic constriction of the infraorbital nerve to establish the TN model.The electroacupuncture group received deep electroacupuncture for 14 days after modeling for 14 days.The BME403 filament method was used to monitor the pain threshold of rats in each group,the expression of p-ERK and ERK protein was detected by immunohistochemistry,and the levels of Kv3.4 and Kv4.3 mRNA were detected by real-time fluorescent quantitative PCR(qPCR).Results There was no significant difference in the pain threshold of the three groups of rats 1 day before modeling(P>0.05).7 days after modeling,the pain threshold of the model group was higher than that of the sham operation group(P<0.05).On the 14th day after modeling,the pain threshold of rats in the model group was lower than that in the sham operation group(P<0.05),but there was no significant difference between the electroacupuncture group and the model group(P>0.05).The pain threshold of the electro-acupuncture group was higher than that of the model group on the 7th and 14th day after treatment(P<0.05).14 days after treatment,the p-ERK and ERK immunofluorescence intensity values of the model group were significantly higher than those of the sham operation group,and the electroacupuncture group was significantly lower than that of the model group(P<0.05).14 days after treatment,Kv3.4mRNA and Kv4.3mRNA in the model group were lower than the sham operation group,and the electroacupuncture group was higher than the model group(P<0.05).Conclusion Deep electroacupuncture treatment of TN can increase the pain threshold,and its mechanism is related t
