不同剂量右美托咪定麻醉在子宫肌瘤腹腔镜手术中的应用分析

摘　　要：目的探讨右美托咪定不同剂量麻醉在子宫肌瘤腹腔镜手术中的应用效果。方法选取2017年11月至2020年11月间湖南省妇幼保健院收治的800例子宫肌瘤患者,采用随机数字表法分为观察组和对照组,每组400例。两组患者均行子宫肌瘤腹腔镜手术,观察组患者采用单次静脉滴注右美托咪定麻醉,对照组患者采用持续静脉泵注右美托咪定麻醉,比较两组患者不良反应发生情况、麻醉前后心率情况和相关时间指标。结果观察组患者拔管时间、麻醉复苏室停留时间和定向力恢复时间均短于对照组,差异均有统计学意义(均P<0.05)。观察组患者不良反应总发生率为2.0%,低于对照组的16.3%,差异有统计学意义(P<0.05)。拔管时及拔管后5min,观察组患者心率和平均动脉压均低于对照组,差异均有统计学意义(均P<0.05)。结论子宫肌瘤患者行腹腔镜手术期间采用单次静脉滴注右美托咪定麻醉,可降低心率和平均动脉压,缩短拔管和苏醒时间,降低术后不良反应发生率,值得推行。Objective To probe into the efficacy of different doses of dexmedetomidine as an anesthetic agent in laparoscopic surgery of hysteromyoma.Methods Eight hundred patients with hysteromyoma who received treatment at Hunan Maternal and Child Health Care Hospital were selected from November2017 to November 2020.They were randomly divided into an observation group and a control group with 400 patients in each group.Both groups underwent laparoscopic surgery for hysteromyoma.The observation group was given single dose intravenous dexmedetomidine and the control group was given continuous pump infusion of dexmedetomidine.Adverse reactions,heart rate before and after anaesthesia and relevant time indices were compared between the two groups.Results The extubation time,length of stay at anesthesia recovery room and directional power recovery time were shorter in the observation group than in the control group(all P<0.05).The overall incidence of adverse reactions was 2.0%in the observation group which was lower than 16.3%of the control group(P<0.05).At extubation and 5 min after extubation,the average heart rates and arterial pressure were lower in the observation group than in the control group(all P<0.05).Conclusion Single dose intravenous dexmedetomidine as an anesthetic agent during laparoscopic surgery can significantly reduce heart rate and average arterial pressure,shorten extubation time and recovery time,and effectively reduce the incidence of postoperative adverse reactions in patients with hysteromyoma.It is worthy of promotion.
