摘　　要：[目的 ]分析上海市嘉定区2009-2018年卫生总费用核算结果,探讨卫生筹资变化情况,为完善区域卫生筹资决策提供依据。[方法 ]采用卫生总费用来源法,对嘉定区的政府卫生支出、社会卫生支出和个人现金卫生支出等指标开展时间序列分析,并对嘉定区和4个直辖市2018年卫生筹资结果进行比较。[结果 ] 2009-2018年上海市嘉定区卫生筹资总额逐年增长,2018年卫生总费用占GDP的比重为5.14%,略低于上海市(6.81%)和全国平均水平(6.36%)。从筹资构成变化趋势看,社会卫生支出是拉动卫生费用上涨的主要因素。2018年,社会医疗保障支出占社会卫生支出的75.32%。个人卫生支出占比总体呈逐年下降趋势。[结论 ]上海市嘉定区卫生筹资总额持续提高,筹资结构逐步优化,区域内卫生筹资体系建设有待进一步完善。Objective This research aims to analyze the results of total health expenditure accounting from 2009 to 2018 in Jiading district of Shanghai, discuss the change of health financing so as to provide basis for perfecting regional health policy-decision. Methods It used total health expenditure source method to carry out time series analysis on the indexes of government, society and personal cash health expenditure in Jiading district, and compared health financing result among Jiading district and 4 municipalities in 2018. Results From 2009 to 2018, the total amount of health financing in Jiading district had increased year by year. The proportion total health expenditure in Jiading GDP was 5.14% in 2018, which was slightly less than that in Shanghai(6.81%) and nationwide average level(6.36%). From the perspective of the financing structure trends, social health expenditure was the main factor of pulling the rising of total health expenditure. The proportion of social health insurance expenditure accounted 75.32% of social health expenditure in 2018. The proportion of personal health expenditure totally showed down trend year by year. Conclusions The total amount of health financing in Jiading district is sustained increasing, and the financing structure has been gradually optimized. Intra-regional health financing system construction has yet to be further improved.
