摘  要:目的探讨[^(18)F]-氟代脱氧葡萄糖(^(18)F-FDG)正电子发射型计算机断层显像与计算机断层扫描(PET/CT)最大标准化摄取值(SUV max)与乳腺浸润性导管癌临床病理特征的相关性。方法回顾性分析58例乳腺浸润性导管癌患者的临床资料,所有患者术前均行^(18)F-FDG PET/CT显像。采用Pearson相关分析SUV max与病灶大小及Ki-67水平的关系,采用协方差分析不同病理参数[雌激素受体(ER)、孕激素受体(PR)、人表皮生长因子受体2(HER2)及Ki-67]、不同病理亚型、不同转移特征乳腺癌患者SUV max的差异。结果乳腺癌病灶SUV max与病灶大小、Ki-67水平均呈正相关(均P<0.05)。ER、PR阴性者的SUV max高于阳性者,HER2阴性及不确定者的SUV max低于HER2阳性者(均P<0.05)。4种病理亚型乳腺癌患者的SUV max差异有统计学意义,其中Luminal A型的SUV max低于非Luminal A型(均P<0.05)。有同侧腋窝淋巴结转移和远处转移者SUV max分别高于无同侧腋窝淋巴结转移和无远处转移者(均P<0.05)。结论^(18)F-FDG PET/CT显像SUV max增高与乳腺癌侵袭性病理亚型及转移有关,可为乳腺癌治疗方案的选择及预后评估提供影像学依据。Objective To investigate the correlation of maximum standardized uptake value(SUV max)of^(18)F-fluorodexoxyglucose(18 F-FDG)position emission tomography(PET)/CT with clinicopathological characteristics in breast invasive ductal carcinoma.Methods The clinical data of 58 patients with breast invasive ductal cancer were analyzed retrospectively,and all patients received^(18)F-FDG PET/CT imaging before operation.Pearson correlation analysis was applied to the relationship of SUV max with focal size and Ki-67 level.Covariance analysis was performed on the differences in SUV max among breast cancer patients with different pathological parameters(estrogen receptor[ER],progesterone receptor[PR],human epidermal growth factor receptor 2[HER2],and Ki-67),with different pathological subtypes,or with different metastasis features.Results SUV max of breast cancer lesions positively correlated with focal size and Ki-67 level(all P<0.05).Subjects with ER or PR negative had higher SUV max than positive subjects(P<0.05),subjects with HER2 negative or uncertain subjects experienced decreased SUV max than subjects with HER2 positive(all P<0.05).Patients with breast cancer of four different pathological subtypes exhibited statistically significant difference in SUV max,among them,Luminal A breast cancer had lower SUV max than non-Luminal A breast cancer(all P<0.05).Subjects with ipsilateral axillary node metastases and distal metastases had higher SUV max than those without ipsilateral axillary node metastases and distal metastases respectively(all P<0.05).Conclusion The increased SUV max of^(18)F-FDG PET/CT imaging is related to the invasive pathological subtype and metastasis of breast cancer,which provides the imaging basis for treatment regimen option and prognosis evaluation in breast cancer.
