大黄免煎颗粒经营养管鼻饲在胃癌术后加速康复中的应用

摘　　要：目的探讨大黄免煎颗粒经营养管鼻饲在胃癌术后加速康复中的应用。方法将2017年1月—2019年1月在焦作市第二人民医院普外一区行胃癌根治术的住院患者共70例纳入本研究。按照随机数字表法分为观察组(n=35),对照组(n=35)。其中对照组采用禁食、胃肠减压、抑酸、抗感染及对症支持治疗;观察组在对照组的基础上,应用大黄免煎颗粒经营养管鼻饲,观察两组术后腹胀、恶心呕吐、吻合口瘘、肠梗阻发生率,并对比两组肛门排气时间、C反应蛋白(CRP)恢复时间、术后住院时间、术后营养餐应用时间。结果观察组术后腹胀、恶心呕吐、吻合口瘘、肠梗阻发生率低于对照组;肛门排气时间、CRP恢复时间、术后住院时间短于对照组;术后营养餐应用时间早于对照组(P<0.05)。结论大黄免煎颗粒鼻饲配合传统的方法应用于胃癌术后患者能够明显缩短患者胃肠功能恢复时间,降低术后并发症发生率,促进患者术后肠功能恢复,缩短住院时间,达到加速康复目的。Objective To explore the application of rhubarb Decoction-free granule in tube nasal feeding for rapid recovery of gastric cancer after operation.Methods This study included 70 inpatients who underwent radical gastrectomy from January 2017 to January 2019 in area 1 of General Surgery of Jiaozuo the Second People’s Hospital. According to the method of random number table, the patients were divided into observation group(n=35) and control group(n=35). In the control group, fasting, gastrointestinal decompression, anti-acid, anti-infection and symptomatic support were used;In the observation group, on the basis of the control group, Rhubarb free-decoction granules were used for feeding through nutrient tube and nasal feeding, the incidence of postoperative abdominal distension, nausea and vomiting, anal exhaust time, CRP recovery time, postoperative hospital stay, Anastomotic Fistula, intestinal obstruction, postoperative nutritional meal application time were observed. Results The anal exhaust time, CRP recovery time, hospitalization time, anastomotic leakage and intestinal obstruction rate in the observation group were significantly lower than those in the control group(P<0.05), and the application time of nutritional meals was earlier than that of the control group(P>0.05).Conclusion The early application of rhubarb-free granules in the early postoperative period of gastric cancer combined with traditional methods can significantly shorten the recovery time of gastrointestinal function, reduce the incidence of postoperative complications, promote the recovery ofpostoperative intestinal function, shorten the length of hospital stay, and achieve rapid rehabilitation.
