内镜下组织粘合剂注射联合套扎对食管胃底静脉曲张破裂出血的疗效及并发症分析

摘　　要：目的分析内镜下组织粘合剂注射联合套扎对食管胃底静脉曲张破裂出血的疗效及并发症。方法回顾性分析100例食管胃底静脉曲张破裂出血患者临床资料,依治疗方法不同分为对照组和治疗组,各50例。对照组患者采取传统套扎治疗,治疗组患者采取内镜下组织粘合剂注射联合套扎治疗。比较两组患者止血成功率、6个月再出血率及疼痛评分,临床疗效及并发症发生情况。结果两组患者止血成功率、治疗前疼痛评分比较差异无统计学意义(P>0.05);治疗组6个月再出血率4.00%优于对照组的20.00%,治疗后疼痛评分(5.11±0.31)分低于对照组的(6.45±0.55)分,差异具有统计学意义(P<0.05)。治疗组患者的治疗总有效率98.00%高于对照组的80.00%,并发症发生率4.00%低于对照组的20.00%,差异具有统计学意义(P<0.05)。结论内镜下组织粘合剂注射联合套扎术治疗食管胃底静脉曲张破裂出血的效果较好,并发症发生率较低,再出血发生率低,效果显著,值得推广。Objective To analyze the efficacy and complications of endoscopic tissue adhesive injection combined with ligation in the treatment of esophageal and gastric variceal bleeding.Methods A total of 100 patients with esophageal and gastric variceal bleeding were divided into control group and treatment group according to different treatment methods,with 50 cases in each group.The control group was treated with traditional ligation,and the treatment group was treated with endoscopic tissue adhesive injection combined with ligation.The success rate of hemostasis,6-month rebleeding rate,pain score,clinical efficacy and occurrence of complications were compared between the two groups.Results Before treatment,there was no statistically significant difference in success rate of hemostasis and pain score between the two groups(P>0.05).The 6-month rebleeding rate 4.00%of the treatment group was better than 20.00%of the control group,and pain score after treatment(5.11±0.31)points was lower than(6.45±0.55)points of the control group.All the difference was statistically significant(P<0.05).The total effective rate 98.00%of the treatment group was higher than 80.00%of the control group,and the incidence of complications 4.00%was lower than 20.00%of the control group.All the difference was statistically significant(P<0.05).Conclusion Combination of endoscopic tissue adhesive injection and ligation shows remarkable effect in the treatment of esophageal and gastric variceal bleeding with low incidence of complications and rebleeding rate,and it is worthy of promotion.
