灭幽汤联合西药治疗幽门螺杆菌感染消化性溃疡临床效果

摘　　要：目的观察灭幽汤联合西药治疗幽门螺杆菌(Hp)感染消化性溃疡临床效果.方法选取2018年1月-2020年1月湖北医药学院附属随州医院收治的H p感染消化性溃疡患者100例,按照随机数表法将其分为研究组和对照组,各50例.对照组单用西药标准三联疗法(治疗2周),研究组在对照组西药治疗的基础上予以灭幽汤治疗1个月.停药后复查胃镜和^(14)C尿素呼气试验(^(14)C-UBT)进行疗效评价,统计两组患者治疗前后胃脘疼痛、反酸暖气、胸胁胀闷、口干口苦、食少纳差症状差异,分析中医症候疗效及创面愈合效果,比较两组治疗前后炎症指标、胃酸分泌功能指标变化和不良反应.结果研究组中医症候疗效总有效率为92.00%和创面愈合总有效率88.00%均显著高于对照组(P<0.05);治疗后两组中医症候积分、血清与胃液炎症因子白细胞介素-6(IL-6)、IL-8水平和胃酸分泌功能指标均显著降低,且研究组指标水平均低于对照组(P<0.05);研究组患者出现3例头疼头晕、3例胃肠道不适,对照组出现1例头疼头晕、2例胃肠道不适和1例皮疹,患者均可耐受且未影响治疗.结论Hp感染消化性溃疡患者采用灭幽汤联合西药治疗可明显提高Hp根除率,促进溃疡愈合,改善临床症状及炎症反应,且无明显毒副作用,临床效果优于单用西药治疗.OBJECTIVE To observe the clinical effect of Mieyou Decoction combined with western medicine on treatment of peptic ulcer induced by Helicobacter pylori(Hp)infection.METHODS A total of 100 patients with Hp infection peptic ulcer who were treated in the Suizhou Hospital Affiliated to Hubei College of Medicine form Jan 2018 to Jan 2020 were enrolled in the study and randomly divided into the study group and the control group,with 50 cases in each group.The control group was treated with the standard triple therapy of western medicine for 2 weeks,while the study group was given additional Mieyou Decoction for 1 month on basis of the treatment of the control group.Gastroscopy and ^(14)C-UBT were carried out to evaluate the curative effect after withdrawal of drugs,the symptoms such as epigastric pain,acid reflux heating,chest tightness,dry throat with bitter taste and poor appetite were statistically analyzed before and after the treatment,the curative effect of TCM symptoms and wound healing effect were evaluated,and the levels of inflammatory indexes,gastric acid secretion indexes and incidence of adverse reactions were observed and compared between the two groups before and after the treatment.RESULTS The total effective rate of TCM symptoms of the study group was 92.00%,significantly higher than that of the control group;the total effective rate of wound healing of the study group was 88.00%,significantly higher than that of the control group(P<0.05).The scores of TCM syndromes,levels of interleukin-6(IL-6)and IL-8 in serum and gastric acid and indexes of gastric acid secretion function of both groups were significantly reduced after the treatment,and the levels of above indexes of the study group were significantly lower than those of the control group(P<0.05).There were 3 cases of headache and dizziness and 3 cases of gastrointestinal discomfort in the study group and 1 case of headache and dizziness,2 cases of gastrointestinal discomfort and 1 case of rash in the control group.All of the patients were tolerant
