口服扶正升白汤对大肠癌术后化疗白细胞计数及miR-125b和miR-124表达的影响

摘　　要：目的探究扶正升白汤对大肠癌术后化疗患者白细胞(WBC)计数及miR-125b、miR-124表达水平的影响.方法选取2018年3月-2019年7月天津中医药大学第二附属医院肿瘤科收治大肠癌术后化疗致白细胞缺乏患者84例,采用简单数表法随机分为对照组和研究组,每组各42例,对照组患者采用口服地榆升白片和鲨肝醇片治疗,研究组采用扶正升白汤治疗,治疗周期均为4周;比较两组患者WBC计数、免疫功能指标及外周血miR-125 b和miR-124水平变化,观察两组临床疗效及不良反应.结果研究组疾病控制有效率为78.57%,高于对照组的52.38%(χ^(2)=6.372,P=0.012);治疗后,两组患者WBC计数和T淋巴细胞亚群CD_(4)^(+)、CD_(4)^(+)/CD_(8)^(+)、免疫球蛋白A(IgA)、IgG、IgM及miR-124水平均高于治疗前,T淋巴细胞亚群CD_(8)^(+)和miR-125b水平均低于治疗前(P<0.05),且研究组指标改善幅度优于对照组(P<0.05).治疗期间,两组患者黏膜损伤、胃肠道反应、肝肾功能损伤、白细胞减少等不良反应发生率比较差异有统计学意义(χ^(2)=15.899,P<0.001).结论扶正升白汤可有效控制大肠癌术后化疗所致白细胞减少,并通过降低miR-125b、提高miR-124表达提高化疗疗效.OBJECTIVE To explore the effect of Fuzheng Shengbai Decoction on white blood cell(WBC)counts and expression levels of miR-125 b and miR-124 in the colorectal cancer patients after chemotherapy.METHODS Totally 84 colorectal cancer patients with chemotherapy-induced leukopenia who were treated in the department of oncology of the Second Affiliated Hospital of Tianjin University of Traditional Chinese Medicine from Mar 2018 to Jul 2019 were enrolled in the study and randomly divided into the control group and the study group,with 42 cases in each group.The control group was treated with oral administration of Diyu Shengbai tablets and Sharkol tablets,while the study group was given Fuzheng Shengbai Decoction,the treatment cycle of both group was 4 weeks.The WBC counts,immune function indexes and peripheral blood miR-125 b and miR-124 were compared between the two groups of patients,and the clinical effect and adverse reactions of the two groups were observed.RESULTS The effective rate of control of disease of the study group was 78.57%,significantly higher than 52.38%of the control group(χ^(2)=6.372,P=0.012).The levels of WBC,T lymphocyte subsets CD_(4)^(+),CD_(4)^(+)/CD_(8)^(+),immunoglobin A(IgA),IgG,IgM and miR-124 of the two groups of patients were significantly higher after the treatment than before the treatment(P<0.05),and the above indexes were improve more significantly in the study group than in the control group(P<0.05).There was significant difference in the incidence of adverse reactions such as mucosal damage,gastrointestinal reaction,liver and kidney function damage and leukopenia between the two groups of patients during the treatment(χ^(2)=15.899,P<0.001).CONCLUSION Fuzheng Shengbai Decoction can effectively control the leukopenia induced by chemotherapy and boost the effect of chemotherapy by reducing the miR-125 b and increasing the miR-124.
