摘　　要：目的分析C-反应蛋白(CRP)、转化生长因子-β1(TGF-β1)、miR-146a在幽门螺杆菌(Hp)感染患者中的表达及意义。方法选取2019年3月-2020年4月在福建医科大学附属泉州第一医院门诊部收治的幽门螺杆菌感染患者128例,分为胃溃疡组28例,慢性萎缩性胃炎组50例,胃癌组50例,按不同严重程度的慢性萎缩性胃炎分为轻度18例,中度16例,重度16例,按不同Borrmann分型分为Ⅰ~Ⅱ18例,Ⅲ15例,Ⅳ17例,按不同浸润深度分为未穿透黏膜下层25例,穿透黏膜下层25例,采用酶联免疫法检测CRP、TGF-β1水平,荧光定量PCR法检测miR-146a水平,免疫组化法染色处理,并统计结果。结果与胃溃疡组相比,慢性萎缩性胃炎组、胃癌组CRP、TGF-β1、miR-146a表达水平均升高;不同严重程度慢性萎缩性胃炎患者CRP、TGF-β1、miR-146a表达水平比较差异有统计学意义(P<0.05),随病情加重CRP、TGF-β1、miR-146a表达水平均升高;不同Borrmann分型胃癌患者CRP、TGF-β1、miR-146a表达水平比较差异有统计学意义(P<0.05),随病情加重CRP、TGF-β1、miR-146a表达水平均升高;浸润深度穿透黏膜下层胃癌患者CRP、TGF-β1、miR-146a表达水平均高于未穿透黏膜下层患者(P<0.05)。结论CRP、TGF-β1、miR-146a的表达与幽门螺杆菌感染的程度有关,幽门螺杆菌感染的胃溃疡、慢性萎缩性胃炎、胃癌患者中CRP、TGF-β1、miR-146a呈现出高表达,监测CRP、TGF-β1、miR-146a水平有助于评估幽门螺杆菌感染相关疾病患者预后。OBJECTIVE To investigate the expression of C-reactive protein(CRP),transforming growth factor-β1(TGF-β1)and miR-146 a in the patients with Helicobacter pylori infection and analyze the significance.METHODS A total of 128 patients with H.pylori infection who were treated in the outpatient department of the First Affiliated Hospital of Quanzhou Medical University from Mar 2019 to Apr 2020 were enrolled in the study and were divided into the gastric ulcer group with 28 cases,the chronic atrophic gastritis group with 50 cases and the gastric cancer group with 50 cases.The patients with chronic atrophic gastritis were divided into the mild group with 18 cases,the moderate group with 16 cases and the severe group with 16 cases according to the severity,the patients were divided into theⅠ-Ⅱgroup with 18 cases,theⅢgroup with 15 cases and theⅣgroup with 17 cases according to the Borrmann typing,and the patients were divided into the no submucosa penetration group with 25 cases and the submucosa penetration group with 25 cases according to the depth of infiltration.The levels of CRP and TGF-β1 were detected by enzyme-linked immunosorbent assay,the miR-146 a level was detected with the use of fluorescence quantitative PCR,the staining was carried out by means of immunohistochemistry,and the results were statistically analyzed.RESULTS The levels of CRP,TGF-β1 and miR-146 a of the chronic atrophic gastritis group and the gastric cancer group were higher than those of the gastric ulcer group.There were significant differences in the levels of CRP,TGF-β1 and miR-146 a among the patients with different severities of chronic atrophic gastritis(P<0.05);with the aggravation of the illness condition,the expression levels of CRP,TGF-β1 and miR-146 a were elevated;there were significant differences in the expression levels of CRP,TGF-β1 and miR-146 a among the gastric cancer patients with different Borrmann types(P<0.05);with the aggravation of the illness condition,the expression levels of CRP,TGF-β1 and miR-146 a wer
