摘 要:目的探讨幽门螺杆菌(Hp)感染对胃癌组织microRNA-19a(miR-19a)表达的影响。方法选择2018年5月-2020年5月在南阳医学高等专科学校第一附属医院接受手术治疗的135例胃癌患者,根据是否存在Hp感染,分为Hp阳性组(n=87)和Hp阴性组(n=48)。记录患者年龄、肿瘤直径、分化程度、TNM分期、浸润深度和淋巴结转移等临床特征。术前检测血清白细胞介素6(IL-6)、IL-8、肿瘤坏死因子α(TNF-α)、C-反应蛋白(CRP)水平。荧光定量PCR检测癌组织miR-19a水平,蛋白印迹法(WB)检测Ki67、周期蛋白依赖性激酶1(CDK1)、基质金属蛋白酶2(MMP-2)、金属蛋白酶组织抑制因子(TIMP-2)、核因子κB(NF-κB)和磷酸化NF-κB(p-NF-κB)蛋白表达。结果胃癌患者Hp阳性与肿瘤TNM分期、浸润深度和淋巴结转移有关(P<0.05);Hp阳性组患者癌组织和癌旁组织中miR-19a水平均高于Hp阴性组(P<0.05),且癌组织中miR-19a水平与肿瘤TNM分期、浸润深度和淋巴结转移有关(P<0.05);Hp阳性组患者血清IL-6、IL-8、TNF-α和CRP水平均高于Hp阴性组(P<0.05),癌组织中Ki67、CDK1、MMP-2、p-NF-κB/NF-κB蛋白均高于Hp阴性组(P<0.05),TIMP-2蛋白低于Hp阴性组(P<0.05)。结论Hp感染可以促进miR-19a表达,激活NF-кB信号通路,促进胃癌细胞的增殖和侵袭,促进胃癌的恶性进展。OBJECTIVE To explore the effect of Helicobacter pylori(Hp)infection on expression of miR-19 a in gastric cancer tissues.METHODS A total of 135 gastric cancer patients who underwent surgical procedures in the First Affiliated Hospital of Nanyang Medical College from May 2018 to May 2020 were enrolled in the study and divided into the Hp-positive group with 87 cases and the Hp-negative group with 48 cases according to the status of Hp infection.The clinical characteristics of the patients such as age,tumor diameter,differentiation degree,TNM staging,invasion depth and lymph node metastasis were recorded.The levels of serum interleukin-6(IL-6),IL-8,tumor necrosis factor-α(TNF-α)and C-reactive protein(CRP)were detected before the surgery.The miR-19 a level in cancer tissues was detected by means of fluorescence quantitative PCR,and the expression levels of Ki67,cyclin-dependent kinase 1(CDK1),matrix metalloproteinase 2(MMP-2),tissue inhibitor of metalloproteinases-2(TIMP-2),nuclear factorκB(NF-κB)protein and phosphorylated NF-κB(p-NF-κB)were detected by using Western blot.RESULTS The positive Hp in the gastric cancer patients was associated with TNM staging,invasion depth and lymph node metastasis(P<0.05).The level of miR-19 a in cancer tissues and para-cancerous tissues was higher in the Hp-positive group than in the Hp-negative group(P<0.05)and the level of miR-19 a in cancer tissues was related to TNM staging,invasion depth and lymph node metastasis(P<0.05).The levels of serum IL-6,IL-8,TNF-αand CRP of the Hp-positive group were higher than those of the Hp-negative group(P<0.05).The expression levels of Ki67,CDK1,MMP-2 and p-NF-κB/NF-κB proteins in cancer tissues were higher in the Hp-positive group than in the Hp-negative group(P<0.05),and the TIMP-2 protein level of the Hp-positive group was lower than that of the Hp-negative group(P<0.05).CONCLUSION The Hp infection may promote the expression of miR-19 a,activate NF-кB signaling pathway,promote the proliferation and invasion of gastric cancer cells as
