摘  要:目的探讨白细胞介素-18(IL-18)基因多态性与脑胶质瘤易感性的相关性及白细胞分化抗原14(CD14)基因多态性与脑胶质瘤术后颅内感染风险的相关性。方法选取2018年5月-2020年6月南阳市第二人民医院神经外科重症监护室收治的93例脑胶质瘤患者为病例组,健康体检者58名为对照组。采用集合酶链式反应(PCR)比较两组IL-18基因rs549908位点、rs187238位点的基因型分布。病例组患者接受胶质瘤手术,根据是否发生术后颅内感染,分为感染组20例、非感染组73例,分析感染患者CD14基因rs2569190位点、rs3138078位点的基因型分布。结果病例组IL-18基因rs549908位点AA基因型频率高于对照组,AC与CC基因型频率低于对照组(P<0.05);病例组IL-18基因rs187238位点GG、GA、AA基因型频率分布与对照组比较,差异无统计学意义;感染组CD14基因rs2569190位点TT基因型频率高于非感染组(P<0.05)。感染组CD14基因rs3138078位点GG、GT、TT基因型频率分布与非感染组比较,差异无统计学意义。结论IL-18基因rs549908位点的AA基因型与脑胶质瘤易感性有关,CD14基因rs2569190位点的TT基因型与胶质瘤术后颅内感染有关。OBJECTIVE To explore the correlation between interleukin-18(IL-18)gene polymorphisms and susceptibility to brain glioma and the correlation between cluster of differentiation antigen(CD14)gene polymorphisms and risk of postoperative intracranial infection.METHODS A total of 93 patients with brain glioma who were treated in intensive care unit of neurosurgery department of Nanyang Second People′s Hospital from May 2018 to Jun 2020 were assigned as the case group,and 58 healthy people who received physical examination were set as the control group.The genotypes at IL-18 gene rs549908 locus and rs187238 locus were compared between the two groups by using polymerase chain reaction(PCR).The patients of the case group received glioma surgery and were divided into the infection group with 20 cases and the non-infection group with 73 cases according to the status of postoperative intracranial infection.The genotypes at CD14 gene rs2569190 locus and rs3138078 locus in the patients with infection were observed.RESULTS The AA genotype frequency at IL-18 gene rs549908 locus of the case group was significantly higher than that of the control group,and frequencies of AC and CC genotypes of the case group were significantly lower than those of the case group(P<0.05).There were no significant differences in the frequencies of GG,GT and TT genotypes at IL-18 gene rs187238 locus between the case group and the control group;the frequency of TT genotype at CD14 gene rs2569190 locus of the infection group was significantly higher than that of the non-infection group(P<0.05);there were no significant differences in the frequencies of GG,GT and TT genotypes at CD14 gene rs3138078 locus between the infection group and the non-infection group.CONCLUSION The AA genotype at IL-18 gene rs549908 locus is associated with the susceptibility to brain glioma,and the TT genotype at CD14 gene rs2569190 locus is associated with the postoperative intracranial infection.
