特殊组织类型乳腺癌的影像学表现和病理分析

摘　　要：目的分析几种特殊组织类型乳腺癌的影像学表现及病理学机制。方法收集23例特殊组织类型乳腺癌(导管内癌、黏液癌、浸润性小叶癌、导管内乳头状癌、浸润性微乳头状癌)的影像学资料,观察其钼靶、CT及超声等影像学征象,并与病理结果进行对照分析。结果特殊组织类型乳腺癌23例,右乳13例,左乳10例,其中导管内癌9例,黏液癌5例,浸润性小叶癌5例,导管内乳头状癌2例,浸润性微乳头状癌2例。钼靶诊断符合率82.6%,超声诊断符合率91.3%,所有患者CT图像上相应部位均可发现乳腺内病变,三者结合诊断符合率100%。各种类型的乳腺癌均有其典型的或不典型的影像学表现。结论特殊组织类型的乳腺癌影像学表现复杂,影像学结果综合分析可提高其诊断准确性。Objective To study the imaging findings and pathological mechanism of several special types of breast cancer.Methods The imaging data of 23 special types of breast cancer(intraductal carcinoma,mucinous carcinoma,meullary carcinoma,invasive lobular carcinoma,intraductal papillary carcinoma,invasive micropapillary carcinoma)were collected,and the imaging findings of mammography,CT and ultrasound were observed and compared with pathological findings.Results There were 23 cases of special type breast cancer,13 cases of right breast and 10 cases of left breast,including 9 cases of intraductal carcinoma,5 cases of mucinous carcinoma,5 cases of invasive lobular carcinoma,2 cases of intraductal papillary carcinoma and 2 cases of invasive micropapillary carcinoma.The coincidence rate of mammography diagnosis and ultrasonography diagnosis was 82.6%and 91.3%,respectively.All patients could find breast lesions in the corresponding parts of CT images.The coincidence rate of combined diagnosis was 100%.All types of breast cancer have their typical or atypical imaging findings.Conclusion The imaging manifestations of special types of breast cancer are complex.Comprehensive analysis of imaging results can improve the diagnostic accuracy of breast cancer.
