摘  要:目的比较结直肠肿瘤预测评分系统、数值化免疫法联合粪便DNA检测法(fecal immunochemical test and DNA,FIT-DNA)、噗噗管在结直肠癌及癌前病变筛选中的差异。方法选择2019年1月—12月本院消化内科174名就诊且接受结直肠癌预测评分表评测,同时完成粪便检测以及肠镜检查的患者作为研究对象,根据肠镜及病理结果将上述病例资料分为正常组、结直肠癌组、高风险腺瘤组、低风险腺瘤组。分别进行结直肠肿瘤预测评分系统、FIT-DNA、噗噗管及肠镜检查,并分析前三种方法对结直肠癌及癌前病变的诊断差异。结果FIT-DNA对结直肠癌、高风险腺瘤的诊断敏感率达78%,对结直肠癌的敏感率达100%;病灶体积≥2 cm时,FIT-DNA敏感率达100%,优于肿瘤预测评分系统与噗噗管。结论FIT-DNA可作为一种无创的、有效的结直肠癌及其癌前病变筛查手段,肿瘤预测评分系统联合噗噗管可以提高结直肠癌及其癌前病变的诊断敏感率。Objective To compare the differences of colorectal cancer prediction score system,numerical immunoassay combined with fecal immunochemical test and DNA(FIT-DNA)and puff tube in the screening of colorectal cancer and precancerous lesions.Methods From January 2019 to December 2019,174 patients who went to the department of gastroenterology those received the examination of colorectal cancer prediction score system,FIT-DNA,and colonoscopy were enrolled as study subjects.According to the results of enteroscopy and pathology,the patients were divided into normal control group,colorectal cancer group,high risk adenoma group,and low risk adenoma group.All the patients received the detection of colorectal cancer prediction score system,FIT-DNA,puff tube,and colonoscopy,and the differences of the first three methods in the diagnosis of colorectal cancer and precancerous lesions were analyzed.Results The sensitivity rate of FIT-DNA in the diagnosis of colorectal cancer and high-risk adenoma was 78%,and the sensitivity rate of FIT-DNA in the diagnosis of colorectal cancer was 100%;the sensitivity rate of FIT-DNA was 100%when the lesion size was greater than or equal to 2cm,which was better than the tumor prediction score system and poop poop tube.Conclusions FIT-DNA could be used as a noninvasive and effective screening method for colorectal cancer and its precancerous lesions.Tumor prediction score system combined with puff tube could improve the diagnostic sensitivity of colorectal cancer and its precancerous lesions.
