摘  要:目的探讨钙黏蛋白23(CDH23)与甲苯诱导听力损失之间的相关性。方法2019年10—12月对深圳市南山区某印刷厂进行职业卫生学和流行病学调查,选择其企业工人作为研究对象,均衡年龄、工龄、高血压以及中耳疾患等因素,将其分为3组作为实验组(甲苯暴露组、噪声暴露组和甲苯与噪声混合暴露组),选择同企业办公室白领作为对照组;通过高效液相色谱法测定尿液中的马尿酸水平以分析甲苯内暴露量;运用纯音听阈测试分析研究对象听力水平;通过二代测序法分析CDH23基因突变位点。结果甲苯暴露组、噪声暴露组、甲苯噪声混合暴露组和对照组中体内马尿酸含量分别为(269.91±297.92)、(178.08±200.37)、(315.69±232.60)和(123.01±123.94)μg/mL,甲苯暴露组和甲苯噪声混合暴露组马尿酸的含量均高于对照组(均P<0.05);甲苯暴露组、噪声暴露组、甲苯噪声混合暴露组和对照组中纯音听阈值分别为(19.33±5.14)、(18.42±7.61)、(20.71±8.19)和(12.27±3.27)dB,实验组纯音听阈值均高于对照组(均P<0.01),甲苯噪声混合暴露组纯音听阈值高于与噪声暴露组(P<0.01);甲苯暴露组中,CDH23基因rs1227049(G/C)及rs1227051(G/A)位点的突变频率均与纯音听阈值相关(均P<0.05)。结论甲苯职业暴露可加重噪声暴露所致的听力损失,且甲苯所致的听力损失可能与CDH23 rs1227049及rs1227051位点突变频率相关。Objective To investigate the correlation between cadherin 23(CDH23)and toluene induced hearing loss.Methods The occupatio nal hygiene and epidemiological survey was performed in a printing enterprises in Nanshan District of Shenzhen from October to December 2019,and the workers of which were divided into three groups as experimental groups(toluene exposure group,noise exposure group,toluene and noise mixed exposure group)by balancing factors such as age,working age,hypertension and middle ear disease,meanwhile with white-collar workers in the same corporate office as a control group.The amount of exposure in toluene was analyzed by urine hippuric acid levels using high performance liquid chromatography.Pure tone audiometry test was used to analyze the hearing level of the subjects.CDH23 gene mutation sites was analyzed by next generation sequencing method.Results The hippuric acid content in the toluene exposure group,noise exposure group,toluene and noise mixed exposure group and control group was(269.91±297.92),(178.08±200.3),(315.69±232.60)and(123.01±123.94)μg/m L,respectively.The hippuric acid content in the toluene exposure group,and toluene and noise mixed exposure group was higher than that in the control group(both P<0.05).The pure tone hearing thresholds in the toluene exposure group,noise exposure group,toluene and noise mixed exposure group and control group was(19.33±5.14),(18.42±7.61),(20.71±8.19)and(12.27±3.27)dB,respectively.The pure tone hearing threshold of the experimental groups was higher than that of the control group(all P<0.01),and the pure tone hearing threshold of the toluene and noise mixed exposure group was higher than that in the noise exposure group(P<0.01).In the toluene exposure group,the mutation frequency of rs1227049(G/C)and rs1227051(G/A)on CDH23 was correlated with pure sound hearing threshold(both P<0.05).Conclusion Occupational exposure to toluene can aggravate hearing loss caused by noise exposure,and hearing loss caused by toluene may be associated with the mutati
