腰方肌阻滞在腹腔镜胆囊切除术中的应用

摘　　要：目的探讨B超引导下腰方肌阻滞联合舒芬太尼对腹腔镜胆囊切除术患者术后镇痛效果及安全性。方法将2018年6月—2019年6月我院收治的80例将行腹腔镜胆囊切除术患者随机分为对照组和观察组,各40例;术后给予对照组患者舒芬太尼静脉自控镇痛;观察组患者术后在对照组基础上加用B超引导腰方肌阻滞镇痛。比较两组患者术毕即刻、术后2 h、术后12 h、术后24 h视觉模拟疼痛(VAS)评分和不良反应发生率。结果两组术毕即刻VAS评分差异无统计学意义(P>0.05),观察组术后2 h、术后12 h、术后24 h的VAS评分均低于对照组(P<0.05)。观察组不良反应发生率(10.00%)低于对照组不良反应发生率(17.50%),差异无统计学意义(P>0.05)。结论B超引导下腰方肌阻滞联合舒芬太尼镇痛可提高患者腹腔镜胆囊切除术后镇痛效果,且安全可靠。Objective To investigate the analgesic effect and safety of B-ultrasound-guided quadratus lumbar muscle block combined with sufentanil in patients undergoing laparoscopic cholecystectomy.Methods 80 patients with laparoscopic cholecystectomy in our hospital from June 2018 to June 2019 were randomly divided into control group and observation group,40 cases in each group;patients in control group were given sufentanil patient-controlled analgesia after operation;patients in observation group were given lumbar quadratus block analgesia guided by B-ultrasound after operation.Visual analogue pain(VAS)scores was compared between the two groups immediately after operation,2 hours after operation,12 hours after operation and 24 hours after operation.Results There was no significant difference in VAS score and incidence of adverse reactions between the two groups(P>0.05).The VAS scores at 2 h,12 h and 24 h after operation in the observation group were significantly lower than those in the control group(P<0.05).Adverse reactions in the observation group(10.00%)were lower than those in the control group(17.50%),but no significant difference was found(P>0.05).Conclusion B-ultrasound-guided lumbar quadratus muscle block combined with sufentanil can significantly improve the analgesic effect of laparoscopic cholecystectomy,and it is safe and reliable.
