基于复杂协同度模型我国公立医院与医疗保险协同关系研究:以2009-2018年截面数据为例

摘　　要：目的:通过研究公立医院医疗服务供给子系统与医疗保险子系统的协同性,为国内公立医院改革提供政策参考。方法:利用复杂协同度模型对两个子系统协同性进行分析。结果:公立医院医疗服务供给子系统和医疗保险子系统10年有序度呈指数上升趋势,且处于低水平协同状态,最低值出现在2012年(0.0122),最高值出现在2018年(0.6768)。结论:医疗保险子系统对公立医院医疗服务供给子系统的贡献在明显增加,在解决个人就医负担方面发挥了重要作用。因此,既要肯定医疗保险在公立医院复杂系统中的重要作用,同时也要通过政策措施(加大公立医院财政补贴力度、成立医疗集团整体上市拓展筹资渠道等),盘活公立医院资本(包括人力资本和实物资本),发挥公立医院自身动力,减轻医疗保险基金的压力。Objective:To study the synergy between the medical service supply subsystem and the medical insurance subsystem in public hospitals to provide policy reference for the reform of public hospitals in China.Methods:The cooperation between two subsystems was analyzed by using the complex collaboration model.Results:The public hospital medical service supply subsystem and the medical insurance subsystem showed an exponential upward trend in 10 years,and were in a low level of coordination and in the low level of synergy.The lowest value appeared in 2012(0.0122),and the highest value appeared in 2018(0.6768).Conclusion:The contribution of China’s medical insurance subsystem to the medical service supply subsystem of public hospitals is increasing in detail and plays an important role in solving the burden of individual medical treatment.Therefore,we should not only affirm the important role of medical insurance in the complex system of public hospitals,but also adopt policy measures(e.g.increasing financial subsidies in public hospitals,or setting up an overall listing of medical groups to raise funds to expand funding channels),invigorating the capital of public hospitals(including human capital and physical capital),giving play to the power of public hospitals themselves,and reducing the pressure of medical insurance funds in China.
