前哨淋巴结活检手术治疗乳腺癌患者的有效性及对拔管时间、总引流量的影响

摘　　要：目的:探讨前哨淋巴结活检手术治疗乳腺癌患者的有效性及对拔管时间、总引流量的影响。方法:选取2019年1-12月本院收治的乳腺癌患者40例。将患者随机分成研究组和对照组,每组20例。对照组给予腋窝淋巴结清扫术(ALDN)治疗,研究组给予前哨淋巴结活检手术(SLN)治疗。比较两组术后6个月患侧上肢肿胀程度、免疫功能指标变化情况、并发症发生情况以及各项临床指标等。结果:术后随访6个月,研究组患侧上肢的肿胀程度明显轻于对照组,差异有统计学意义(P<0.05)。术后6个月,研究组CD3+、CD4+、CD8+水平均优于对照组(P<0.05)。研究组并发症发生率低于对照组,差异有统计学意义(P<0.05)。研究组手术时间、拔除引流管时间均短于对照组,术中出血量及总引流量均少于对照组,差异均有统计学意义(P<0.05)。结论:乳腺癌患者应用前哨淋巴结活检手术具有理想效果,能减少并发症的发生,减轻上肢肿胀程度,有效缩减引流管拔除时间,改善患者免疫功能,可推广使用。Objective:To investigate the effectiveness of sentinel lymph node biopsy in the treatment of breast cancer patients and its effect on extubation time and total drainage volume.Method:A total of 40 breast cancer patients admitted to our hospital from January to December 2019 were selected.The patients were randomly divided into study group and control group,20 cases in each group.The control group received axillary lymph node dissection(ALDN),and the study group received sentinel lymph node biopsy(SLN).The degree of upper limb swelling,the changes of immune function indexes,the incidence of complications and various clinical indicators 6 months after operation were compared between two groups.Result:After 6 months of follow-up,the swelling degree of the affected upper limb in the study group was significantly lighter than that in the control group,the difference was statistically significant(P<0.05).6 months after operation,the levels of CD3+,CD4+and CD8+in the study group were better than those in the control group(P<0.05).The complication rate in the study group was lower than that in the control group,the difference was statistically significant(P<0.05).The operation time and drainage tube removal time of the study group were shorter than those of the control group,and the intraoperative blood loss and total drainage volume were less than those of the control group,the differences were statistically significant(P<0.05).Conclusion:Sentinel lymph node biopsy in the treatment of breast cancer patients has an ideal effect,which can reduce the incidence of complications,reduce the degree of upper limb swelling,effectively reduce the drainage tube removal time,improve the immune function of patients,and can be popularized.
