摘  要:目的分析人类表皮生长因子受体-2(HER-2)、原癌基因增殖细胞核抗原Ki-67和细胞外基质金属蛋白酶诱导因子CD147表达对乳腺癌病情进展、预后生存的评估价值。方法选取2014年7月—2017年6月诊治的乳腺癌101例,收集乳腺癌组织及癌旁正常组织(距离癌组织5 cm以上),分别作为乳腺癌组和癌旁组织组。比较两组HER-2、Ki-67及CD147的表达,观察三者与临床病理参数的关系。记录所有患者在随访时间内存活及死亡情况,采用多因素Logistic回归分析影响乳腺癌患者预后生存的危险因素。结果HER-2、Ki-67及CD147在乳腺癌组中阳性表达率均高于癌旁组织组(P<0.01)。乳腺癌患者HER-2、Ki-67及CD147表达水平与组织分化类型、TNM分期、脉管侵袭和淋巴结侵袭具有显著相关性(P<0.05,P<0.01)。截至随访结束,101例乳腺癌患者生存65例,死亡36例,病死率为35.64%。组织中低分化、TNM分期为Ⅲ+Ⅳ期、脉管侵袭、淋巴结侵袭、HER-2、Ki-67及CD147表达阳性是影响乳腺癌患者预后生存的独立危险因素(P<0.05,P<0.01)。结论HER-2、Ki-67及CD147在乳腺癌组织中表达异常,且与患者病情进展及预后生存密切相关,或可作为预测乳腺癌患者预后生存的分子标志物和肿瘤治疗的潜在靶点。Objective To analyze values of expressions of human epidermal growth factor receptor 2(HER-2),nuclear-associated antigen Ki-67 and extracellular matrix metalloproteinase inducer CD147 in evaluation of disease progression and prognostic survival of patients with breast cancer.Methods A total of 101 patients with breast cancer who had been diagnosed and treated during July 2014 and June 2017 were selected,and their breast cancer tissue and adjacent normal tissue(more than 5 cm away from the cancer tissues)were collected and used as breast cancer group and adjacent tissue group respectively.Expressions of HER-2,Ki-67 and CD147 were compared between two groups,and correlations between expressions of the three indicators and clinicopathological parameters were observed.Conditions of survival and death of all patients during the follow-up were recorded.Multiple Logistic regression was used to analyze risk factors affecting prognostic survival of patients with breast cancer.Results Positive expression rates of HER-2,Ki-67 and CD147 proteins in breast cancer group were significantly higher than those in adjacent tissue group(P<0.01).Levels of HER-2,Ki-67 and CD147 were significantly correlated with tissue differentiation type,tumour node metastases(TNM)stage,vascular invasion and lymph node invasion in patients with breast cancer(P<0.05,P<0.01).At the end of the follow-up among 101 breast cancer patients,65 patients survived,and 36 patients died,and the fatality rate was 35.64%.Low-to-moderate tissue differentiation,TNMⅢ+Ⅳstaging,vascular invasion,lymph node invasion,and positive expressions of HER-2,Ki-67 and CD147 were independent risk factors affecting prognostic survival of breast cancer patients(P<0.05,P<0.01).Conclusion HER-2,Ki-67 and CD147 are abnormally expressed in breast cancer tissues,and they are closely associated with patients'disease progression and prognostic survival,which may be used as molecular markers in prediction of prognostic survival and as potential targets for tumor therapy of patients wit
