急性缺血性脑卒中患者NLR与凝血功能相关性研究

摘　　要：目的探讨急性缺血性脑卒中患者血液中的中性粒细胞和淋巴细胞比值(NLR)与凝血功能的相关性。方法选取2019年1—5月在天津市环湖医院就诊的急性缺血性脑卒中患者40例作为脑梗组,选取同期体检健康志愿者30例作为健康组,采用血细胞分析检测中性粒细胞数(NEU#)和淋巴细胞数(LYM#),采用血凝分析仪检查凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、凝血酶时间(TT)和纤维蛋白原(FIB)水平,并分析急性缺血性脑卒中患者NLR的变化以及与凝血功能的相关性。结果脑梗组NLR和FIB为4.41±1.79和(23.28±4.01)g/L,明显高于健康组的2.11±1.35和(2.92±0.70)g/L,APTT和TT明显低于健康组,差异均有统计学意义(P<0.05)。NLR水平与APTT和TT水平呈负相关,与FIB水平呈正相关。结论急性缺血性脑卒中患者存在NLR异常升高,NLR水平与APTT、FIB和TT水平均具有相关性,NLR可作为潜在的凝血功能诊断指标。Objective To investigate the correlation between the neutrophil-to-lymphocyte ratio(NLR)in blood of patients with acute ischemic stroke and coagulation function.Methods From January to May in2019,40 patients with acute ischemic stroke in Tianjin Huanhu Hospital were selected as cerebral infarction group,and 30 healthy volunteers were selected as healthy group.Neutrophil count(NEU#)and lymphocyte count(LYM#)were detected by blood cell analysis.Prothrombin time(PT),activated partial thromboplastin time(APTT),thrombin time(TT)and fibrinogen(FIB)were detected by hemagglutination analyzer.The changes of NLR in patients with acute ischemic stroke and its correlation with coagulation function were analyzed.Results NLR and FIB in cerebral infarction group were 4.41±1.79 and(23.28±4.01)g/L,significantly higher than 2.11±1.35 and(2.92±0.70)g/L in healthy group,APTT and TT were significantly lower than those in healthy group,and the differences were statistically significant(P<0.05).NLR levels were negatively correlated with APTT and TT levels,and positively correlated with FIB levels.Conclusion The patients with acute ischemic stroke had abnormal increase of NLR.NLR level was correlated with APTT,FIB and TT levels.NLR could be used as a potential diagnostic index of coagulation function.
