急性房颤患者病因及诱因分析

摘　　要：目的探讨急性房颤住院患者的病因及诱因。方法对天津医科大学总医院滨海医院2017年1月—2019年12月出院的83例急性房颤患者的临床资料进行回顾性分析。结果83例急性房颤的病因统计为冠心病61例、高血压病58例、心力衰竭57例、糖尿病21例、慢性阻塞性肺疾病14例、慢性肾功能衰竭5例、甲亢型心脏病3例、原发性心肌病3例、风湿性心脏病1例,合并诱因情况主要构成为感染36例、心力衰竭失代偿19例、急性冠脉综合征8例、高血压亚急症3例、低钾血症3例、甲亢1例等。结论在临床处理急性房颤患者的过程中,应注意患者是否合并与房颤相关的,尤其是以上常见的病因及诱因。及早发现诱因及合并的基础疾病,并对其进行针对性的治疗,可以及早控制急性房颤患者的心室率,尽快改善患者症状,以避免漏诊情况的发生,使急性房颤患者得到及时全面的救治。Objective To explore the etiology and inducement of hospitalized patients with acute atrial fibrillation.Methods The clinical data of 83 patients with acute atrial fibrillation discharged from Binhai Hospital,General Hospital of Tianjin Medical University from January 2017 to December 2019 were retrospectively analyzed.Results The etiologies of 83 cases of acute atrial fibrillation were 61 cases of coronary heart disease,58 cases of hypertension,57 cases of heart failure,21 cases of diabetes,14 cases of chronic obstructive pulmonary disease,5 cases of chronic renal failure,3 cases of hyperthyroid heart disease,3 cases of primary cardiomyopathy,1 case of rheumatic heart disease,combined with the inducements mainly consist of 36 cases of infection,19 cases of decompensation of heart failure,8 cases of acute coronary syndrome,3 cases of hypertensive urgencies,3 cases of hypokalemia,1 case of hyperthyroidism,and so on.Conclusion In the clinical treatment of patients with acute atrial fibrillation,attention should be paid to whether the patient was associated with atrial fibrillation,especially the common etiologies and inducements mentioned above.Early detection of inducements and associated underlying diseases and targeted treatments could control the ventricular rate of patients with acute atrial fibrillation as soon as possible,improve the symptoms of patients as soon as possible,and avoid missed diagnosis,so that patients with acute atrial fibrillation could get timely and comprehensive treatment.
