微创穿刺血肿引流术联合开颅血肿清除术对高血压性脑出血伴脑疝患者的疗效观察

摘　　要：目的:探讨微创穿刺血肿引流术(MIPHD)联合开颅血肿清除术对高血压性脑出血(HICH)伴脑疝患者近期疗效及远期预后的影响。方法:选取2018年1月—2019年12月在我院接受治疗的HICH合并脑疝患者62例,随机分为对照组和观察组各31例。对照组采用开颅血肿清除术进行治疗,观察组采用MIPHD联合开颅血肿消除术。结果:术后NIHSS评分和MBI评分观察组改善程度显著高于对照组,差异有统计学意义(P<0.05);有效率分别为观察组83.87%,对照组70.97%,差异有统计学意义(P<0.05);术后1年,观察组生活质量显著高于对照组,差异有统计学意义(P<0.05),而且观察组患者预后分级情况优于对照组,差异有统计学意义(P<0.05)。结论:MIPHD联合开颅血肿消除术在HICH合并脑疝患者的治疗中有效改善患者整体预后情况,显著提高患者生活质量。Objective To investigate the effect of MIPHD combined with craniotomy and hematoma clearance on HICH patients with cerebral hernia and analyze the correlation between postoperative death.Methods From January 2018 to December 2019,62 patients with hich complicated with cerebral hernia were randomly divided into control group and observation group;The control group was treated with craniotomy hematoma removal,and the observation group was treated with MIPHD combined with craniotomy hematoma removal.Results The improvement of NIHSS score,MBI score and effective rate in the observation group was significantly higher than the control group(P<0.05);One year after operation,the quality of life in the observation group was higher than the control group(P<0.05),and the prognosis grading in the observation group was better than the control group(P<0.05).Conclusion MIPHD combined with craniotomy in the treatment of HICH patients with cerebral hernia can effectively improve the overall prognosis of patients and significantly improve the quality of life of patients.
