加速康复外科护理对老年胃癌患者术后康复效果影响的Meta分析

摘　　要：目的:通过M e t a分析评价加速康复外科护理对老年胃癌患者术后康复效果的影响。方法:计算机检索Cochrane Library、PubMed、Embase、Web of Science、知网、万方和维普数据库中将加速康复外科护理用于老年胃癌患者的随机对照试验。检索时限均为建库至2019年3月,由2名研究者对所获文献进行独立筛查、质量评价和资料提取,并应用Revman 5.3软件进行分析。结果:纳入15篇文献,共1304例患者,其中加速康复外科护理为试验组(653例),常规护理为对照组(651例)。Meta分析结果显示,加速康复外科护理与常规护理相比可缩短老年胃癌患者住院时间、降低住院费用、加速机体功能恢复、显著降低并发症整体发生率(P<0.05),但第1次经口进食时间无缩短、出血率、感染率等的差异无统计学意义(P>0.05),但亚组分析结果显示肺部感染率显著降低(P<0.05)。结论:加速康复外科护理可促进老年胃癌患者术后康复,缩短住院时间,降低住院费用,且不增加并发症发生率,但需制定更具个性化加速康复外科护理方案。Objective To evaluate the effect of accelerated rehabilitation surgical nursing on postoperative rehabilitation of elderly patients with gastric cancer through Meta analysis.Methods Randomized controlled trials of accelerated rehabilitation surgical nursing in elderly patients with gastric cancer were searched in Cochrane Library,PubMed,Embase,Web of Science,CNKI,Wanfang and VIP databases.The retrieval time was from the establishment of the database to March 2019.Two researchers conducted independent screening,quality assessment and data extraction of the collected literatures,and analyzed them with RevMan 5.3 software.Results A total of 1304 patients were included in 15 literatures,among which accelerated rehabilitation surgery nursing was the experimental group(653 cases)and routine nursing was the control(651 cases).Meta analysis,according to the results of accelerated recovery compared with routine nursing can shorten surgical nursing of elderly patients with gastric cancer hospital stay,reduce hospitalization expenses,accelerate the body function recovery,reduce the overall incidence of complications(P<0.05),but no obviously shorten the first through the mouth eating time,bleeding,infection and so on the differences of no statistical significance(P>0.05),but subgroup analysis result in pulmonary infection rate(P<0.05).Conclusions Accelerated rehabilitation surgical nursing can promote the postoperative rehabilitation of elderly patients with gastric cancer,shorten the length of stay,reduce the cost of hospitalization,and do not increase the incidence of complications,but it is necessary to develop more personalized accelerated rehabilitation surgical nursing plan.
