摘　　要：目的探讨保留乳头乳晕复合体(NAC)的乳腺皮下腺体全切术(NSM)在早期乳腺癌中应用的疗效。方法将2017年1月至2021年5月南京医科大学第一附属医院收治的69例确诊为早期乳腺癌患者分为传统手术组(保乳组或者单纯切除组)和NSM组,比较手术时间、术后拔管时间、术后并发症、术后乳房触压感觉、复发率、美观度和满意度。结果NSM组手术时间比传统手术组更短(P<0.05),术后拔管时间无统计学差异(P>0.05)。保乳组乳房NAC和胸部皮肤触觉保留比NSM组更好(P<0.05),但是NSM组总体满意度更高(P<0.05)。3组患者随访至今均无复发或转移。结论与传统的保乳术或单纯乳房切除术相比,NSM应用于早期乳腺癌患者有同等的安全疗效和更高的满意度。Objective To investigate the efficacy of nipple-areola complex(NAC)sparing mastectomy(NSM)with in early breast cancer.Methods From January 2017 to May 2021,69 patients diagnosed with early breast cancer in our hospital were divided into traditional surgery group(breast-conserving or simple mastectomy)and NSM group.The time of operation procedure and postoperative extubation,postoperative complications,breast pressure sensation,recurrence rate,aesthetics and satisfaction were compared.Results The operation time of the NSM group was shorter than that of the traditional operation group(P<0.05),and there was no significant difference in the postoperative extubation time(P>0.05).The skin tactile preservation of NAC and breast in the breast-conserving group were better than those in the NSM group(P<0.05),but the NSM group had higher overall satisfaction(P<0.05).None of the three groups of patients had recurence or metastasis.Conclusions Compared with traditional breast-conserving surgery or simple mastectomy,NSM has the same safety and efficacy and higher satisfaction for early breast cancer patients.
