单中心5年间肾癌手术病例资料小结与分析

摘　　要：目的分析肾癌临床特点及根治性肾切除术(RN)与肾部分切除术(PN)围手术期指标的差异。方法统计2016—2020年宝鸡市中心医院泌尿外科收治的肾癌患者的临床资料。对患者年龄、性别、手术方式、出血量、手术时间,术后病理类型、住院时间、治疗费用等指标进行分析。结果宝鸡市中心医院5年共收治569例肾癌患者,男女比例为1.05,发病年龄为(65.00±11.20)岁。其中305例具有完整手术资料,肾透明细胞癌比例为82.30%(251/305),腹腔镜手术占88.52%(270/305)。PN组与RN组的平均术中出血量分别为(36.13±16.37)mL、(118.11±44.55)mL,手术时间分别为(1.95±0.68)h、(2.16±0.85)h,肿瘤最大径分别为(3.10±1.06)cm、(5.28±2.30)cm。结论宝鸡市肾癌病理类型以透明细胞癌为主,腹腔镜手术为主要手术方式。PN术中出血量、手术时间、肿瘤最大径小于根治性肾切除术(P<0.05)。Objective To explore the clinical characteristics of renal cell carcinoma and the differences in perioperative indexes between radical nephrectomy and partial nephrectomy.Methods The clinical data of renal cell carcinoma cases treated in our hospital during 2016 and 2020 were collected.Patients age,gender,operation method,blood loss,operation time,pathological type,hospitalization days and treatment costs were retrospectively analyzed.Results A total of 569 patients were enrolled.The male/female ratio was 1.05.Patients average age was(65.00±11.20)years.Among all cases,305 had complete sur-gical data,82.30%of which were clear cell renal cell carcinoma(251/305),and 88.52%received laparoscopic surgery(270/305).The blood loss,operation time and maximum tumor diameter were(36.13±16.37)mL vs.(118.11±44.55)mL,(1.95±0.68)h vs.(2.16±0.85)h and(3.10±1.06)cm vs.(5.28±2.30)cm for partial nephrectomy and radical nephrectomy,respectively.Conclusion Clear cell carcinoma was the main pathological type of renal cell carcinoma in Baoji City,and laparoscopic surgery was the main operation method.The blood loss,operation time and maximum tumor diameter of partial nephrectomy were less or shorter than those of radical nephrectomy(P<0.05).
