摘　　要：目的探讨CK7、CK5、TTF-1用于肺腺癌和肺鳞癌鉴别的临床价值。方法选取2018年1月至2019年12月本院收治的经肺活检或手术切除的肺腺癌患者45例(肺腺癌组),肺鳞癌患者20例(肺鳞癌组),肺部良性病变患者30例(良性病变组)作为研究对象。患者均接受CK7、CK5、TTF-1检测,比较3组CK7、CK5、TTF-1阳性率。结果肺腺癌组与肺鳞癌组CK5与TTF-1阳性表达率比较差异有统计学意义(P<0.05);肺腺癌组与肺鳞癌组CK7阳性表达率比较差异无统计学意义;良性病变组CK7、CK5、TTF-1均呈阴性表达;3组CK5与TTF-1染色评分比较差异有统计学意义(P<0.05);3组CK-7染色评分比较差异无统计学意义。结论CK7、CK5、TTF-1联合检测可有效鉴别肺腺癌和肺鳞癌,为疾病的诊断提供参考。Objective To explore the clinical value of CK7,CK5,TTF-1 in the differentiation of lung adenocarcinoma and lung squamous cell carcinoma.Methods 45 patients with lung adenocarcinoma(lung adenocarcinoma group),20 patients with lung squamous cell carcinoma(lung squamous cell carcinoma group)and 30 patients with benign lung lesions(benign lesion group)admitted to our hospital from January 2018 to December 2019 were selected as study subjects.Patients were tested for CK7,CK5 and TTF-1,and the positive rates of CK7,CK5 and TTF-1 among the three groups were compared.Results There was significant difference in CK5 and TTF-1 positive expression rate between the lung adenocarcinoma group and the lung squamous cell carcinoma group(P<0.05);there was no significant difference in CK7 positive expression rate between the lung adenocarcinoma group and the lung squamous cell carcinoma group;CK7,CK5 and TTF-1 were negative expression in the benign lesions group;there was significant difference in CK5 and TTF-1 staining scores among the three groups(P<0.05).There was no significant difference in the staining scores of CK-7 among the three groups.Conclusion The combined detection of CK7,CK5 and TTF-1 can effectively distinguish adenocarcinoma and squamous cell carcinoma of the lung and provide reference for the diagnosis of the disease.
