鼻内窥镜下鼻腔泪囊吻合术治疗慢性泪囊炎疗效及治疗满意度临床观察

摘　　要：目的探讨鼻内窥镜下鼻腔泪囊吻合术治疗慢性泪囊炎、疗效及治疗满意度临床观察。方法选取2017年7月—2019年11月在本院行手术治疗的慢性泪囊炎114例为研究对象,根据手术方法不同分为观察组56例和对照组58例。观察组给予鼻内窥镜下鼻腔泪囊吻合术,对照组给予外路鼻腔泪囊吻合术。比较2组手术指标、临床疗效、疗效检测指标、溢泪程度评分、美观度、术后并发症发生情况及患者满意度。结果观察组手术时间及住院时间均短于对照组,术中出血量少于对照组(P<0.01)。观察组总有效率明显高于对照组(P<0.05)。与术前比较,2组术后1、3个月SchirmerⅠ试验长度及BUT试验时间明显下降,且观察组术后1个月上述指标及术后3个月Schirmer试验长度低于对照组,差异有统计学意义(P<0.05);但2组术后3个月BUT试验时间比较差异无统计学意义(P>0.05)。与术前比较,2组术后6个月溢泪程度评分显著降低,且观察组低于对照组(P<0.05)。与对照组比较,观察组术后2、6个月泪道通畅率均显著增高,复发率显著降低,差异有统计学意义(P<0.05)。与对照组比较,观察组并发症总发生率及瘢痕发生率明显降低,治疗后总满意度明显升高,差异有统计学意义(P<0.05)。结论慢性泪囊炎患者实施鼻内窥镜下鼻腔泪囊吻合术,临床疗效好,且该手术方法安全性高、患者术后满意度高。Objective To explore clinical efficacy of nasal dacryocystorhinostomy under nasal endoscope in treatment of patients with chronic dacryocystitis and clinical observation on therapeutic degree of satisfaction.Methods A total of 114 patients with chronic dacryocystitis who had undergone surgery during July 2017 and November 2019 were enrolled in this study,and the patients were divided into observation group(n=56)and control group(n=58)according to different surgical approaches.Observation group underwent nasal dacryocystorhinostomy under nasal endoscope,while control group underwent external dacryocystorhinostomy.Values of surgical indexes,clinical efficacy,indexes of efficacy detection,score of epiphora degree,aesthetics,incidence rate of postoperative complications and satisfaction degree were compared between two groups.Results In observation group,operative duration and length of hospital stay were significantly shorter than those in control group,while intraoperative volume of blood loss was less than that in control group(P<0.01).The total effective rate in observation group was significantly higher than that in control group(P<0.05).Compared with those before surgery,values of SchirmerⅠtest length and BUT test time in the 1 st and 3 rd months after operation were significantly decreased in two groups,and the values in the 1 st month and SchirmerⅠtest length in the 3 rd months after operation in observation group were significantly lower than those in control group(P<0.05),but there was no significantly difference in BUT test score in the 3 rd months between two groups(P>0.05).In the 6 rd months after treatment,scores of epiphora degree were significantly lower than those before treatment in two groups,and the score in observation group was significantly lower than that in control group(P<0.05).Compared with those in control group,patency rates of lacrimal duct in the 2 nd and 6 th months were significantly increased,while recurrence rate was significantly lower in observation group(P<0.05).Compared with
