乳腺肌上皮瘤的影像学及病理学分析

摘　　要：目的分析乳腺肌上皮瘤(AME)的影像学及病理学表现,提高对本病的认识,提高诊断正确率。方法将2014年1月至2019年1月诊断的4例乳腺良性肌上皮瘤为研究对象。结果本组4例AME,发生于左乳3例、右乳1例,大小从1.50 cm×0.90 cm至3.70 cm×3.05 cm不等。影像学表现为单发肿块1例,多结节融合肿块2例,片团状影1例。4例AME钼靶及超声表现形态均不规则,边界清楚3例、边界模糊1例,部分肿块边缘有分叶征及周围可见增粗血管征象,4例均表现为低回声、其内超声均未见丰富血流信号,1例后方回声增强,2例AME超声有局部导管迂曲扩张表现。结论乳腺AME是罕见的乳腺肿瘤,虽临床及影像学表现缺乏特异性,但表现为边界清楚并且周围导管迂曲扩张的多结节型乳腺肿瘤应考虑到AME的可能性,确诊需依靠组织病理学。Objective To analyze the imaging and pathological manifestations of breast benign adenomyoepithe-lioma,so as to improve the understanding of the disease and the diagnostic accuracy.Methods Four cases were diagnosed as subjects from January 2014 to January 2019.Results Of 4 cases with AME there were 3 cases in left breast and 1 case in right breast,with sizes ranging from 1.50 cm×0.90 cm to 3.70×3.05 cm.Imaging findings showed 1 single mass,2 multi-nodule fused masses and 1 lumpy shadow.Four cases with AME mammography and ultrasound manifestation was irregular,boundary clear in 3 cases,1 case with fuzzy boundaries,the margin of some masses showed lobulation signs and visible enlargement vascular signs,4 cases were characterized by low ultrasonic echoes,its rich blood flow signal was not found,1 case with posterior acoustic enhancement,2 case with AME ultrasound has a partial catheter migration expansion performance.Conclusion AME is a rare tumor of the mammary gland.Although the clinical and radiological manifestations are lack of specificity,However,the possibility of AME should be considered for polynoidous breast tumors presenting with well-defined borders and tortuous distention of surrounding ducts,its diagnosis depends on histopathology.
