一例接种卡介苗后导致皮肤结核的病例报告

摘　　要：目的:调查某病例接种卡介苗后发生皮肤结核等疾病与注射疫苗之间的关联性。方法:开展个案调查,收集疫苗受种方、接种单位、疫苗生产企业等相关资料,组织开展疑似预防接种反应调查,形成调查诊断报告。结果:患儿接种卡介苗后4月余,接种部位出现破溃、不愈合现象,并逐渐加重,检查显示IgE轻度升高,基因结果提示STAT 3基因突变,超声示同侧腋下淋巴结肿大,局部组织分泌物可见抗酸杆菌阳性,给予抗结核药物治疗。预防接种过程符合操作规程,疫苗质量合格。结论:接种BCG后发生的皮肤结核等疾病与注射卡介苗有关,属于预防接种异常反应。OBJECTIVE To investigate the association between immunization with Bacille Calmette Guerin(BCG) and skin tuberculosis with other diseases. METHODS A case investigation was carried out to collect the relevant data of recipient,inoculation unit,vaccine manufacturers and other information. Investigation and diagnosis were carried out about immunizationabnormal reaction(Adverse events following immunization,AEFI),and the investigation diagnosis was formed. RESULTS More than 4 months after the child was inoculated with BCG vaccine,the inoculation site appeared ulceration,non-union,and gradually worsened.The examination showed that a slight increase of IgE. The genetic results suggested STAT3 gene mutation.Ultrasound showed ipsilateral axillary lymph node enlargement,local tissue secretions were positive for acid-fast bacilli. Anti-tuberculosis drugs were given.The vaccination process complies with the operating procedures and the vaccine quality was qualified. CONCLUSIONS Kin tuberculosis with other diseases that occurred after BCG vaccination were related to BCG injection,which was an rare adverse reaction.
