电子直肠乙状结肠镜检查在社区老年居民直肠肿物筛查中的应用

摘　　要：为评价直肠乙状结肠镜检查在社区老年居民直肠肿物筛查中的作用,针对北京市西城区西长安街地区4782名老年居民展开筛查,填写危险因素评估调查问卷完成初筛,对高危人群进行直肠乙状结肠镜复筛。并按检出肿物情况分为直肠肿物组472例和对照组1840例。对直肠肿物的危险因素进行单因素和多因素统计分析。结果显示,共检出各类肛肠疾病2275例,其中直肠良性肿物458例(19.8%)、疑似直肠癌17例(0.7%)。经单因素分析性别、身体质量指数(BMI)、糖尿病、结直肠息肉家族史、结直肠癌家族史、吸烟史可能为影响直肠肿物发生发展的危险因素。经多因素分析,性别、BMI、糖尿病史、结直肠息肉家族史、结直肠癌家族史、吸烟史是直肠肿物发病的独立危险因素。结果表明,电子直肠乙状结肠镜检查能有效从目标人群中检出直肠肿物患者,适用于社区老年居民进行直肠肿物的大范围筛查。且操作简便、检出率高、依从性好,在社区筛查应用中具有重要意义。This study was to evaluate the role of electron anosigmoidoscopy to screen rectal neoplasms of communitys’senior residents,screened 4728 senior residents from the region of Xichang’an street:1)primary screen through filling in the questionnaire for investigating and evaluating risk factors;2)re-screen:performing electro-anosigmoidoscopy on high-risk population;3)classification according to the detected neoplasms status,and grouping:neoplasms group(472 cases)and control group(1840 cases);4)risk factors analysis by using univariate analysis and multivariate analysis.As results,the totalled detected various coloproctologcial diseases were 2775 cases,including rectal begin neoplasms of 458 cases(19.8%),suspected rectal cancer of 17 cases(9.7%);Uinvariate analysis showed gender,BMI,DM,family-history of colorectal polyposis,family-history of colorectal cancer,and smoking history could be the risk factors influencing rectal neoplasms developed and progressed;while multi-variate analysis showed above-mentioned risk factors were independent ones inducing that neoplasms invaded.Results show that electron-anosigmoidoscopy can effectively inspect out that patients with rectal neoplasms,therefore is suitable to large-scope screening community’s residents’rectal neoplasms.
