摘 要:目的探讨特发性全身性癫痫(IGE)脑微观结构改变,分析其与癫痫患者生活质量调查表-31(QOLIE-31)的相关性.方法招募20例IGE患者和20例性别、年龄及教育程度相匹配健康对照组(HC组),采集3D T1 BRAVO、T2液体衰减反转恢复(FLAIR)和扩散峰度成像(DKI)数据并评定癫痫患者QOLIE-31得分.计算DKI各参数图,行双样本t检验,比较组间差异,采用Pearson分析方法对差异脑区与QOLIE-31评分进行相关性分析.结果癫痫组与HC组相比,性别、年龄及受教育年限差异无统计学意义(P>0.05).与HC组相比,其中左侧辅助运动区、左额上回、左中央旁小叶、左侧颞横回、右侧颞横回各向异性分数(FA)减低;右侧舌回、右梭状回扩散峰度FA(FAK)升高;双侧海马轴向峰度(AK)降低;右额中回、左辅助运动区、右岛叶、左前扣带回、右海马旁回、右梭状回、右顶下缘角回、左丘脑、左颞横回、右颞横回、左颞上回径向峰度(RK)降低;右额中回、右岛叶、左前扣带回、左后扣带回、双侧海马、右海马旁回、右楔叶、右梭状回、右角回、左丘脑、双侧颞横回、左侧颞上回平均峰度(MK)降低.相关性分析发现,左侧颞横回MK值与QOLIE-31评分呈正相关(r=0.52,P=0.018).结论DKI可定量反映IGE患者多个不对称脑区微观结构改变,MK及RK较其他参数更敏感.DKI对理解IGE病理生理变化及潜在治疗靶区选择可能提供更多影像学依据.Objective To investigate the brain microstructure changes in patients with idiopathic generalized epilepsy(IGE),and to analyze the correlation between brain microstructure and quality of life in epilepsy inventory-31(QOLIE-31).Methods 20 IGE patients and 20 healthy controls(HC)matched with sex,age and education were collected.The data of 3D Ti BRAVO,T2 fluid attenuated inversion recovery(FLAIR)and diffusion kurtosis imaging(DKI)were collected and the QOLIE-31 scores of epileptic patients were evaluated.The parameter maps of DKI were calculated and double-sample t test was performed to compare the differences between groups.Pearson analysis was used to analyze the correlation between different brain regions and QOLIE-31 scores.Results There was no significant difference m sex,age and years of education between epilepsy group and HC group(P>0.05).Compared with HC group,fractional anisotropy(FA)in the left supplementary motor area,left superior frontal gyrus,left paracentral lobule and bilateral heschl gyrus decreased;FA of kurtosis(FAK)in the right lingual gyrus and right fusiform gyrus increased;axial kurtosis(AK)in bilateral hippocampus decreased;radial kurtosis(RK)in the right middle frontal gyrus,left supplementary motor area,right insular lobe,left anterior cingulate gyrus,right parahippocampal gyrus,right fusiform gyrus,right inferior parietal angular gyrus,left thalamus,bilateral heschl gyrus and left superior temporal gyrus decreased;mean kurtosis(MK)in the right middle frontal gyrus,right insular lobe,left anterior cingulate gyrus,left posterior cingllate gyrus,bilateral hippocampus,right parahippocampal gyrus,right cuneate lobe,right fusiform gyrus,right angular gyrus,left thalamus,bilateral heschl gyrus and left superior temporal gyrus decreased.Correlation analysis showed that there was a positive correlation between MK and QOLIE score in the left heschl gyrus(r=0.52,P=0.018).Conclusion DKI can quantitatively reflect the microstructure changes of asymmetric brain regions in patients with IGE,in which th
