摘　　要：目的分析原发性肺黏液表皮样癌(PMEC)的CT表现特征,提高本病的诊断水平.方法回顾性分析28例经病理证实的原发性PMEC的CT征象.结果28例原发性PMEC中22例为中央型,6例为周围型.原发性PMEC均表现为单发、分叶状或卵圆形的肿块影,其中4例病灶内见低密度区,4例见空洞,2例见钙化.CT增强扫描16例表现为轻中度强化,4例呈明显强化.23例原发性PMEC伴气道阻塞性改变,23例见支气管征,4例见灶周新月征.结论原发性PMEC多为中央型,表现为单发分叶状或卵圆形肿物,呈轻中度强化,常伴气道阻塞性改变及支气管征.Objective To analyze the CT features of primary pulmonary mucoepidermoid carcinoma(PMEC)to improve the diagnostic accuracy.Methods The CT features of 28 patients with primary PMEC confirmed by pathology were analyzed retrospectively.Results Among the 28 cases,22 cases were central type and other 6 cases were peripheral type.CT showed solitary,lobulated or oval masses with low density areas in 4 lesions,cavity in 4 lesions and calcification in 2 lesions.On contrast-enhanced CT images,16 lesions had mild-moderate enhancement and 4 lesions had obvious enhancement.23 cases of primary PMEC appeared airway obstruction,23 cases appeared positive bronchial sign,and 4 cases appeared the crescent sign around the lesion.Conclusion Primary PMEC is mainly of central type presenting lobulated or oval mass with mild-moderate enhancement,mostly accompanied by airway obstruction and positive bronchial sign.
