深板层角膜移植术治疗感染性角膜炎的效果

摘　　要：目的评价深板层角膜移植术治疗感染性角膜炎的效果。方法回顾性分析2017年8月至2019年10月济南市第二人民医院感染性角膜炎64例(64只眼)的临床资料,根据手术方式分为2组,其中深板层角膜移植术(DALK)31例(31只眼)作为观察组;穿透性角膜移植术(PKP)33例(33只眼)作为对照组。术后随访1年,比较两组的手术后视力、角膜植片透明度及并发症。结果术后1周、2周、1个月及3个月视力(logMPR)及角膜植片透明度两组间比较差异均无统计学意义(P>0.05)。术后6个月及1年观察组视力优于对照组(t=2.568,3.124;P=0.013,0.003);角膜透明度优于对照组(P<0.05)。术后1年观察组排斥率(6.5%,2/31)低于对照组(12.1%,4/33),两组间差异有统计学意义(P=0.040)。结论DALK术后中远期视力及角膜透明度均优于PKP术后。Objective To evaluate the efficacy of deep anterior lamellar keratoplasty in the treatment of infectious keratitis.Methods The clinical data of 64 eyes of 64 patients with infectious keratitis in Jinan Second People’s Hospital from Aug.2017 to Oct.2019 was collected and retrospectively analyzed.All cases were divided into 2 groups based on the operation method.Thirty-one eyes of 31 patients received deep anterior lamellar keratoplasty(DALK)as the observation group,and 33 eyes of 33 patients received penetrating keratoplasty(PKP)as the control group.Postoperative visual acuity,transparency of corneal graft and postoperative complications were compared between the two groups.The follow-up time was 1 year.Results There was no significant difference between the two groups in visual acuity or corneal graft transparency at 1 week,2 weeks,1 month and 3 months after surgery(all P>0.05).The visual acuity(t=2.568,3.124;P=0.013,0.003)and corneal graft transparency(P<0.05)of the observation group were better than those of the control group at 6 months and 1 year after surgery respectively.The rejection rate of the observation group at 1 year after surgery(6.5%,2/31)was better than that of the control group(12.1%,5/33)(P=0.040).Conclusion The visual acuity and corneal graft transparency of medium and long term after DALK are better than PKP,with fewer complications.
