摘 要:目的比较近视者佩戴CRT型和VST型角膜塑形镜角膜损伤程度。方法回顾性病例研究。濮阳市眼科医院2017年1月至2019年10月佩戴角膜塑形镜的近视110例(212只眼),年龄为8~16(11.22±2.08)岁。屈光状态为等效球镜度-4.00~-0.75 D。所有病例根据戴镜型号分为两组。佩戴CRT型镜者49例(90只眼)为A组,佩戴VST型镜者61例(122只眼)为B组。分别检查戴镜时及摘镜后两种型号镜片切线图和角膜染色的情况。结果戴镜后1个月两组之间荧光素图分别在理想配适、偏松或偏紧可接受、偏松或偏紧不可接受、黏附不可接受的分布比例差异无统计学意义(χ^(2)=0.645,P=0.886);角膜地形图切线图分级Ⅰ级、Ⅱ级、Ⅲ级、Ⅳ级两组间差异无统计学意义(χ^(2)=0.191,P=0.979);角膜染色的情况在0级、Ⅰ级、Ⅱ级+Ⅲ级+Ⅳ级差异无统计学意义(χ^(2)=0.111,P=0.946)。结论VST镜片和CRT镜片在戴镜后1个月对角膜的安全性相近。Objective To compare the degree of corneal injury between wearing CRT and VST orthokeratology lens for myopia.Methods This was a retrospective study.A total of 212 eyes of 110 cases of myopia from Jan.2017 to Oct.2019 in the Ophthalmologic Hospital of Puyang wearing orthokeratology lens were selected.The ages were ranged from 8 to 16 years old,with an average of(11.22±2.08)years and the spherical equivalent degree was-4.00~-0.75 D.All patients were divided into two groups based on the types of lenses worn.There were 90 eyes of 49 cases wearing CRT lens and 122 eyes of 61 cases wearing VST lens.The tangent picture and corneal staining with and without wearing the lens were compared between CRT lens and VST lens.Results At 1 month after wearing orthokeratology lens,there was no significant difference in distributing proportion of the fluorescein picture under conditions of ideal fit,acceptable tolerance,unacceptable tolerance,and unacceptable adhesion between VST lens and CRT lens(χ^(2)=0.645,P=0.886).The tangent picture of grade Ⅰ,Ⅱ,Ⅲ and Ⅳ of VST lens were not significantly different from those of CRT lens(χ^(2)=0.191,P=0.979).The difference of corneal stainings in difference of grade 0, and Ⅱ+Ⅲ+Ⅳ between VST lens CRT lens(χ^(2)=0.111,P=0.946).Conclusion After wearing orthokeratology lens for 1 month,the fluorescein picture,tangent picture and corneal safety of VST lens are not significantly different from those of CRT lens.
