玻璃体切除术治疗伴有玻璃体积血的息肉样脉络膜血管病变

摘　　要：目的观察玻璃体切除术(PPV)治疗伴有玻璃体积血的息肉样脉络膜血管病变(PCV)的效果。方法回顾性分析2018年7月至2020年10月郑州大学第一附属医院收治伴有玻璃体积血的PCV 30例(30眼)的临床资料,所有患者进行了玻璃体切除术,其中8眼联合视网膜造孔引流,视网膜激光光凝和硅油填充;其余22眼仅进行单纯玻璃体切除术和手术灌注液眼内填充,视网膜下出血未进行处理。随访3~9个月,观察视力、眼压及眼底形态改变。结果所有患者玻璃体积血均被清除,8眼视网膜下出血顺利排出,其余22眼中2眼发生复发性玻璃体积血,20眼视网膜下出血逐渐吸收减少。末次随访术眼视力较术前明显提高(t=6.295,P<0.01)。结论玻璃体切除术是伴有玻璃体积血的息肉样脉络膜血管病变有效的治疗方式。Objective To observe the efficacy of pars plana vitrectomy(PPV)for the treatment of polypoidal choroidal vasculopathy(PCV)accompanied with vitreous hemorrhage.Methods This was a retrospective study.Thirty eyes of 30 cases with PCV accompanied with vitreous hemorrhage received pars plana vitrectomy from Jul.2018 to Oct.2020 in the First Affiliated Hospital of Zhengzhou University were included.Among them 8 eyes received peripheral artificial retinal hole,laser photocoagulation and silicone oil tamponade.The other 22 eyes without interventional treatment for subretinal hemorrhage underwent vitrectomy and intraocular tamponade with perfusion fluid.The follow-up time was 3-9 months.The visual acuity,intraocular pressure and ocular fundus morphological changes after surgery were observed.Results Vitreous hemorrhage were cleared in 30 eyes.Subretinal hemorrhage in 8 eyes was discharged successfully with interventional treatment.Two eyes of recurrent vitreous hemorrhage in 22 eyes without intervention were treated with PPV again and subretinal hemorrhage was drained out.Subretinal hemorrhage in other 20 eyes absorbed gradually.The best corrected visual acuity was significantly improved in the last follow-up(P<0.01).Conclusion Vitrectomy is an effective treatment for polypoidal choroidal vasculopathy accompanied with vitreous hemorrhage.
