白内障术后弥漫性角膜上皮糜烂治疗效果

摘　　要：目的评估白内障术后弥漫性角膜上皮糜烂不同治疗方法的效果。方法回顾性分析山西省眼科医院2012年1月至2019年12月白内障术后出现弥漫性角膜上皮糜烂17例(17眼)的临床资料。患者按治疗方式分为3组:A组4例(4眼),进行单纯的药物治疗;B组9例(9眼),进行角膜绷带镜联合药物治疗;C组4例(4眼),进行自体血清滴眼液联合角膜绷带镜及其他药物治疗。观察比较3组:眼部刺激症状缓解和消失时间、角膜上皮完全恢复时间及视力恢复时间。结果经治疗后所有患者眼部刺激症状消失,视力恢复,角膜上皮恢复完整。B组患者视力恢复,眼部刺激症状缓解及消失、上皮恢复时间最短,治疗效果好。结论应用角膜绷带镜联合药物治疗白内障术后角膜上皮糜烂效果较好。但对于角膜上皮糜烂严重,迁延不愈者,可以考虑再联合自体血清滴眼治疗。Objective To evaluate the efficacy of different treatment methods for diffuse corneal epithelial erosion after cataract surgery.Methods This was a retrospective case study.The clinical data of 17 eyes of 17 cases with diffuse corneal epithelial erosion after cataract surgery from Jan.2012 to Dec.2019 in Shanxi Eye Hospital were analyzed.These patients were divided into 3 groups based on the treatment method.Group A with 4 eyes of 4 cases received simple drug therapy.Group B with 9 eyes of 9 cases were treated with bandage contact lenses and drug therapy.Group C with 4 eyes of 4 cases were treated with 20%autologous serum eye drops combined with bandage contact lenses and other drugs.The time of relief and disappearance of ocular irritation symptoms,time of complete recovery of corneal epithelium,and time of vision recovery in three groups were observed and compared.Results After treatment,ocular irritation disappeared,visual acuity recovered and corneal epithelium recovered.The patients in group B had good therapeutic efficacy in visual acuity recovery,remission and disappearance of ocular irritation symptoms,and shorter epithelial recovery time.Conclusion The bandage contact lenses and drug has good efficacy for the treatment of epithelial erosion after cataract surgery.But for serious and persistent epithelial erosion,combined autologous serum eye drops are recommended.
