硅油填充眼白内障改良小切口囊外摘出手术探讨

摘　　要：目的探讨硅油填充眼白内障行改良小切口囊外摘出手术技巧。方法回顾性分析南阳市眼科医院2015年5月至2019年5月硅油填充眼白内障166例(170眼)的临床资料。患者均接受改良小切口囊外摘出人工晶状体植入及硅油取出手术,改良方法首先于颞下象限平坦部穿刺,留置灌注套管,暂不接入灌注,再行小切口白内障囊外摘出人工晶状体植入术。接入灌注恢复眼压,然后于颞上、鼻上平坦部穿刺口行标准三通道硅油取出术。术后随访3个月,观察手术效果。结果163眼(95.88%)顺利进行连续环形撕囊;7眼术中撕囊应用囊剪辅助。165眼(97.06%)成功植入人工晶状体。165眼(97.06%)完成硅油抽吸。结论硅油填充眼白内障进行改良小切口囊外摘出人工晶状体植入及硅油抽吸术效果良好。Objective To investigate the surgical technique of small incision extracapsular cataract extraction for silicone oil-filled eyes with cataract.Methods The clinical data of 170 eyes of 166 cases of silicone oil-filled eyes with cataract from May 2015 to May 2019 in Nanyang Eye Hospital were retrospectively analyzed.All patients underwent modified small incision cataract extracapsular extraction intraocular lens implantation and silicone oil removal.The modified surgical methods were as follows:first,the flat puncture was performed in the inferior temporal quadrant,and the perfusion cannula was indwelled.No perfusion was inserted temporarily,and then small-incision extracapsular cataract extraction and intraocular lens implantation were performed.Intraocular pressure was restored by perfusion,followed by standard three-channel silicone oil removal at the superior temporal and nasal flat punctures.Results There were 163 eyes(95.88%)underwent continuous annular capsulorhexis successfully;7 eyes were assisted by capsule scissors during capsulorhexis.There were 165 eyes(97.06%)underwent intraocular lens implantation successfully;and 165 eyes(97.06%)underwent silicone oil aspiration.Conclusion The modified small incision extracapsular extraction with intraocular lens implantation and silicone oil removal is effective for cataract of silicone oil-filled eyes.
