不同倾斜角的人工晶状体对术后后囊浑浊的影响

摘　　要：目的比较植入不同倾斜角的人工晶状体对老年性白内障术后后囊浑浊的影响。方法回顾性分析南阳南石医院2016年1月至2019年10月老年性白内障行超声乳化术80例(88眼)的临床资料。按术中不同倾斜角分为3个组:A组植入前倾角10°的SKY人工晶状体者27例(31眼);B组植入倾角0°的AO人工晶状体者31例(33眼);C组植入后倾角5°的FLEX人工晶状体者22例(24眼)。比较3组术后前房深度、屈光度、后囊浑浊及并发症发生率。结果术后前房深度3组组间差异无统计学意义(F=1.189,P=0.378)。术后均未出现Ⅲ级后囊浑浊,术后6个月3组Ⅰ级后囊浑浊发生率分别为6.45%(2/31)、9.09%(3/33)、8.33%(2/24),Ⅱ级后囊浑浊发生率分别为6.45%(2/31)、3.03%(1/33)、8.33%(2/24),差异均无统计学意义(P>0.05);屈光度的比较,3组间差异无统计学意义(Z=2.945,P>0.05);3组撕囊直径和术后炎症反应发生情况的比较,均无统计学意义(P>0.05)。所有患者术中及术后均未出现严重并发症。结论老年性白内障植入不同倾斜角的人工晶状体(SKY、AO、FLEX人工晶状体)术后后囊浑浊的发生率无明显差异。Objective To compare the effect of intraocular lens implantation with different inclination angle on occurrence of posterior capsuar opacification of senile cataract patients.Methods The clinical data of 88 eyes of 80 cases with senile cataract who received phacoemulsification from Jan.2016 to Oct.2019 in Nanyang Nanshi Hospital were analyzed retrospectively.All cases were divided into three groups based on different in clination anglea.Group A with SKY lens with 10°anterior inclination angle in 31 eyes,group B with AO lens with 0°anterior inclination angle in 33 eyes,group C with FLEX lens with 5°posterior angle in 24 eyes.The postoperative anterior chamber depth,diopter,posterior capsule opacification and complications were compared among three groups.Results There was no significant difference in the depth of anterior chamber among three groups(F=1.189,P=0.378).The grade III posterior capsule turbidity after operation didn’t appear.The incidence of grade I posterior capsuar opacification was 6.45%(2/31),9.09%(3/33)and 8.33%(2/24)in the three groups at 6 months after operation,and the incidence of grade II posterior capsuar opacification was 6.45%(3/33)3.03%(1/33)and 8.33%(2/24)respectively,and the difference was not statistically significant(P>0.05).There was no significant difference in diopter among the three groups(Z=2.945,P>0.05).There was no significant difference in the diameter of capsulorhexis and postoperative inflammatory reaction among the three groups(P>0.05).There were no serious complication during and after surgery in the three groups.Conclusion There is no difference in the incidence of posterior capsuar opacification in the elderly cataract patients after phacoemulsification and implantation of intraocular lens(SKY,AO,FLEX)with different inclination angle.
