肝动脉化疗栓塞术治疗原发性肝细胞癌预后的影响因素分析

摘　　要：目的探讨肝动脉化疗栓塞术(TACE)治疗原发性肝细胞癌(HCC)预后的影响因素。方法抽取2016年10月至2020年10月于南阳市中心医院接受TACE治疗的HCC患者共296例作为研究对象,回顾性分析其临床资料。计算患者的第1、2、3年累积生存率;并分析影响患者TACE预后的因素。结果本研究共296例患者,男246例,女50例;年龄25~87(56.3±10.8)岁;至随访结束时死亡243例,占82.09%。患者治疗后第1、2、3年累积生存率分别为73.99%、40.20%、22.30%。多因素分析结果显示,肝癌分期、甲胎蛋白(AFP)、谷氨酰转肽酶(GT)、肿瘤最长径、肿瘤数目、动静脉瘘是影响TACE治疗HCC患者预后的独立因素(P<0.05)。结论影响TACE治疗HCC患者预后的独立因素包括肝癌分期、AFP和GT水平、肿瘤最长径、肿瘤数目及动静脉瘘。Objective To investigate the clinical influencing factors of prognosis of patients with primary hepatocellular carcinoma(HCC)treated by transcatheter arterial chemoembolization(TACE).Methods A total of 296 HCC patients who received TACE treatment in Nanyang Central Hospital from October 2016 to October 2020 were selected as the research objects.The clinical data of selected patients were retrospectively analyzed.And the 1-,2-,and 3-year cumulative survival rates were calculated.And the factors influencing the prognosis of patients with HCC treated by TACE were analyzed.Results There were 296 patients in this study,including 246 males and 50 females,aged 25-87(56.3±10.8)years.A total of 243 cases(82.09%)died until to the end of follow-up.The 1-,2-,and 3-year cumulative survival rates were 73.99%,40.20%,and 22.30%,respectively.Multivariate analysis showed that liver cancer stage,α-fetoprotein(AFP),glutamyl transpeptidase(GT),maximum tumor diameter,number of tumors and arteriovenous fistula were independent factors affecting the prognosis of HCC patients treated by TACE(P<0.05).Conclusions The independent prognostic factors of HCC patients treated by TACE are liver cancer stage,AFP and GT levels,maximum tumor diameter,tumor number,and arteriovenous fistula.
