外周血APN、IFN-γ表达与子宫内膜癌患者腹腔镜根治术后复发转移的相关性

摘　　要：目的分析外周血脂联素(APN)、干扰素-γ(IFN-γ)表达与子宫内膜癌患者腹腔镜根治术后复发转移的相关性。方法抽取2017年4月至2018年7月于信阳市中医院接受腹腔镜根治术后发生子宫内膜癌复发转移的患者47例,纳为复发转移组;另抽取同期接受腹腔镜根治术且术后子宫内膜癌未复发转移的患者47例,纳为未复发转移组;对其临床资料进行回顾性分析。全部患者均于术前及出院时接受血清APN、IFN-γ检测,且检测结果资料完整。统计并比较两组相关基线资料及各时点血清APN、IFN-γ水平,分析术前外周血APN、IFN-γ表达与子宫内膜癌患者腹腔镜根治术后复发转移的关系。结果94例子宫内膜癌患者出院时APN水平均较术前升高,IFN-γ水平较术前降低(P<0.05)。复发转移组患者术前糖类抗原125(CA125)、糖类抗原199(CA199)、IFN-γ水平均高于未复发转移组患者,APN水平低于未复发转移组患者(P<0.05)。相关性分析结果显示,血清APN与IFN-γ、CA125、CA199呈负相关(r<0,P<0.05),IFN-γ与CA125、CA199呈正相关(r>0,P<0.05)。经Logistic回归分析显示,血清APN、IFN-γ异常表达与子宫内膜癌患者腹腔镜根治术后复发转移有关,APN低表达、IFN-γ过表达是子宫内膜癌患者腹腔镜根治术后复发转移的风险因子(OR>1,P<0.05)。结论子宫内膜癌患者腹腔镜根治术后复发转移可能与术前血清APN低表达、IFN-γ过表达有关。Objective To analyze the correlation of expression of adiponectin(APN)and interferon-γ(IFN-γ)in peripheral blood with recurrence and metastasis of endometrial carcinoma(EC)after laparoscopic radical resection.Methods A total of 47 patients with recurrence and metastasis of EC after laparoscopic radical resection treated in Xinyang Hospital of Traditional Chinese Medicine from April 2017 to July 2018 were included as the recurrence and metastasis group.Another 47 EC patients without recurrence and metastasis after laparoscopic radical resection at the same period were collected and included as the non recurrence and metastasis group.The clinical data of all selected patients were analyzed retrospectively.Patients all received serum APN and IFN-γtest before operation and at discharge,and the test results of them were complete.The relevant baseline data and serum APN and IFN-γlevels at each time point were counted and compared between the two groups.The relationship of expression of APN and IFN-γin peripheral blood with recurrence and metastasis of EC after laparoscopic radical resection were analyzed.Results The levels of APN in 94 patients with EC at discharge were higher than those before operation,and the level of IFN-γEC at discharge was lower than that before operation(P<0.05).The preoperative carbohydrate antigen 125(CA125),carbohydrate antigen 199(CA199),and IFN-γlevels in the recurrence and metastasis group were higher than those in the non recurrence and metastasis group,and the APN levels were lower than those in the non recurrence and metastasis group(P<0.05).The results of correlation analysis showed that serum APN was negatively correlated with IFN-γ,CA125,and CA199(r<0,P<0.05);IFN-γwas positively correlated with CA125 and CA199(r>0,P<0.05).The results of Logistic regression showed that abnormal expression of serum APN and IFN-γmight be related to the recurrence and metastasis of EC after laparoscopic radical resection,while low expression of APN and over expression of IFN-γmight be risk fac
