齐拉西酮联合氯氮平治疗难治性精神分裂症患者的临床效果

摘　　要：目的探讨齐拉西酮联合氯氮平治疗难治性精神分裂症的临床效果。方法选择我院收治的100例难治性精神分裂症患者为研究对象,时间为2017年2月至2018年11月。随机进行分组,每组50例。单药组单纯给予氯氮平治疗,联合组则采取减量氯氮平联合齐拉西酮治疗。比较两组的临床疗效、阳性和阴性精神症状评定量表(PANSS)评分开始降低的时间、住院时间,治疗前后PANSS评分、简明精神病评定量表(BPRS)评分,不良反应发生情况。结果联合组的治疗总有效率高于单药组(P<0.05)。治疗前,两组PANSS评分、BPRS评分比较无差异(P>0.05);治疗后,联合组PANSS评分、BPRS评分降低幅度大于单药组(P<0.05)。联合组PANSS评分开始降低的时间、住院时间均短于单药组(P<0.05)。联合组头晕、便秘、流涎、体质量增加、脑电图异常等不良反应发生率低于单药组(P<0.05)。结论应用氯氮平联合齐拉西酮治疗难治性精神分裂症患者的效果显著,可有效改善患者的精神病症状,且药物起效更快,不良反应少,安全性高。Objective To investigate the clinical efficacy of ziprasidone combined with clozapine in the treatment of refractory schizophrenia.Methods One hundred patients with refractory schizophrenia from February 2017 to November 1818 were enrolled in our hospital.Randomly,the single-drug group was given clozapine alone,and the combined group was given reduced cisplatin plus ziprasidone.Comparison of the efficacy of two groups of refractory schizophrenia,the time when the Positive and Negative Syndrome Scale(PANSS)score began to decrease,the hospitalization time,the PANSS scores before and after treatment,the Brief Psychiatric Rating Scale(BPRS)scores,and occurrence of adverse reactions.Results The total effective rate of treatment in the combined group was higher than that in the single-agent group(P<0.05).Before treatment,there was no significant difference between the two groups of PANSS scores and the BPRS scores(P>0.05);after treatment,the combined group’s PANSS scores and BPRS scores reduction was greater than that of the single-drug group(P<0.05).The time to decrease the scores of the PANSS scores and the hospitalization time in the combined group were shorter than those in the single-drug group(P<0.05).The incidence of adverse reactions such as dizziness,constipation,salivation,increase in body mass,and abnormal EEG in the combined group was lower than that in the single-drug group(P<0.05).Conclusion Clozapine combined with ziprasidone is effective in the treatment of refractory schizophrenia.It can effectively improve the symptoms of patients,improve the symptoms of psychosis,and has a faster onset of action,with fewer adverse reactions and higher safety.
