摘　　要：为了更好地从中医角度认识脑小血管病(CSVD),笔者回顾分析了古代文献和近代研究中关于CSVD的相关描述,发现CSVD临床症状变化多端,病情复杂,大多数病例并不符合中医脑卒中诊断标准,传统脑卒中的概念难以准确辨识CSVD。根据其发病形势、临床表现及病因病机,建议将CSVD归于“中风”“类中风”(风痱、风癫、风痴、风癔、风眩)“痴呆”“郁病”等范畴。并且,CSVD的发病形式复杂多变,或急性起病、或隐匿起病,或以某一临床症状单独见症、或多个症状兼杂,且其危害性及远期死亡率远高于传统的缺血性脑卒中(大血管卒中),本文即是从中医病名、病程、临床症状角度对CSVD的中医认识及其演变和发展做了初步论述。In order to know cerebral small vessel disease(CSVD)from the perspective of traditional Chinese medicine,the writer makes a retrospective analysis on CSVD in ancient references and modern researches,and finds it changeful in clinical symptoms and complex in patients’condition.Most cases do not meet the diagnostic standards of conventional stroke,which cannot accurately identify CSVD in traditional Chinese medicine.According to its morbidity,clinical manifestations,etiology and pathogenesis,CSVD is suggested to be classified as“stroke”,“apoplectic stroke”(hemiplegia,wind epilepsy,fengchi,apoplectic hysteria,wind dizziness),“dementia”and“depression”.In addition,the onset of CSVD is complicated and various,which may be acute,or hidden,or in a clinical symptom alone,or multiple and miscellaneous symptoms.Its harmfulness is more and the long-term mortality rate is much higher than that of traditional ischemic stroke(large vascular stroke).From the perspective of disease name,course,clinical symptoms,this paper makes a preliminary understanding of traditional Chinese medicine as well as its evolution and development.
