摘  要:目的:探讨HMGA2、IMP3、p16、Ki67在宫颈上皮内瘤变和宫颈癌中的表达及临床预后意义。方法:免疫组化法检测30例正常宫颈组织(NCE)、36例宫颈低级别上皮内病变(LSIL)、38例宫颈高级别上皮内病变(HSIL)和42例宫颈癌(CC)中HMGA2、IMP3、p16及Ki67的表达情况,比较4种分子标记物在不同级别宫颈病变中的表达差异,并分析4种分子标记物之间及其与宫颈癌临床病理特征的关系。结果:HMGA2、IMP3、p16、Ki67阳性表达率与宫颈病变程度呈正相关,差异均有统计学意义(P<0.05)。p16和Ki67阳性表达率在LSIL与HSIL两组比较中,差异有统计学意义(均P<0.05),而HMGA2、IMP3的阳性表达率无统计学意义(均P>0.05)。宫颈癌中HMGA2和IMP3阳性表达呈正相关(r=0.589,P=0.000),p16和Ki67阳性表达呈正相关(r=0.599,P=0.000)。HMGA2表达与宫颈癌患者FIGO分期、分化程度、浸润深度、淋巴结转移、神经浸润显著有关(P<0.05);IMP3表达与宫颈癌患者FIGO分期、分化程度、病理类型、浸润深度、淋巴结转移、脉管浸润、神经浸润有关;Ki67与宫旁浸润显著相关(P<0.05)。结论:HMGA2、IMP3、p16、Ki67均与宫颈癌的发生、发展密切相关。HMGA2和IMP3在促进宫颈癌浸润和转移方面可能具有一定作用,有助于评估宫颈癌患者的不良预后。p16和Ki67对LSIL与HSIL的鉴别诊断具有重要意义。Objective:To investigate the expressions and clinical prognostic significances of HMGA2,IMP3,p16,and Ki67 in cervical intraepithelial neoplasia and cervical cancer.Methods:The expressions of HMGA2,IMP3,p16 and Ki67 were detected by immunohistochemistry in 30 cases of normal cervical tissue,36 cases of low-grade cervical intraepithelial neoplasia(LSIL),38 cases of high-grade intraepithelial neoplasia(HSIL)and 42 cases of invasive cervical cancer(CC).To compare the expression differences of four molecular markers in different grades of cervical lesions and to analyze the correlation between the four molecular markers and the clinical pathological characteristics of cervical cancer.Results:The positive expression rates of HMGA2,IMP3,p16 and Ki67 were positively correlated with the degree of cervical lesions with significant difference(P<0.05).The positive expression rates of p16 and Ki67 were both statistically significant in the comparison between LSIL and HSIL(P<0.05),while HMGA2 and IMP3 were no statistical significance(P>0.05).There was a significant positive correlation between the expression of HMGA2 and IMP3(r=0.589,P=0.000),and p16 expression was positively correlated with Ki67(r=0.599,P=0.000)in cervical cancer tissues.The expression of HMGA2 was significantly related to FIGO stage,differentiation degree,infiltration depth,lymph node metastasis and nerve infiltration in cervical cancer patients(P<0.05).The expression of IMP3 was related to FIGO stage,differentiation degree,pathological type,invasion depth,lymph node metastasis,vascular invasion and nerve invasion in cervical cancer patients.Ki67 was significantly correlated with the periuterine infiltration(P<0.05).Conclusions:HMGA2,IMP3,p16,Ki67 are closely related to the occurrence and development of cervical cancer.HMGA2 and IMP3 promote invasion and metastasis of cervical cancer and are helpful to evaluate the poor prognosis of cervical cancer patients.p16 and Ki67 are of great significance in the differential diagnosis of LSIL and HSIL.
