奥拉西坦联合丁苯酞治疗老年高血压脑出血的临床疗效

摘　　要：目的观察奥拉西坦联合丁苯酞治疗老年高血压脑出血的临床疗效。方法在2017年8月~2020年8月间收治的高血压脑出血患者中抽取110例作为研究对象,照双色球法随机分为两组,蓝色球55例作为对照组,在常规治疗基础上静脉滴注奥拉西坦进行治疗,红色球55例作为研究组,与对照组相同治疗基础上联合静脉滴注丁苯酞氯化钠注射液,比较两组患者治疗前后脑水肿体积、血清SP、Hcy水平、神经功能缺损程度、日常生活能力变化及临床疗效。结果治疗后,两组脑水肿体积、SP与Hcy指标水平、NIHSS评分及ADL评分均改善,差异有统计学意义(P<0.05),且研究组各指标改善程度显著优于对照组,差异有统计学意义(P<0.05);研究组临床治疗总有效率(90.91%)高于对照组(72.73%),差异有统计学意义(χ^(2)=6.111,P=0.013)。结论奥拉西坦联合丁苯酞治疗老年高血压脑出血可显著提高治疗效果,在减轻脑水肿的同时升高SP水平,降低Hcy水平,改善神经功能缺损,提高患者日常生活能力。Objective To observe the clinical efficacy of oxiracetam combined with butylphthalide in the treatment of hypertensive cerebral hemorrhage in the elderly.Methods A total of 110 patients with hypertensive cerebral hemorrhage admitted to our hospital from August 2017 to August 2020 were selected as the research objects and randomly divided into two groups according to the two-color ball method.55 cases of blue ball were choser as the control group.On the basis of treatment,intravenous infusion of oxiracetam was used for treatment.55 cases of red ball were as the study group,combined with intravenous infusion of butylphthalide sodium chloride injection based on the same treatment as the control group,and the volume of brain edema,serum SP,Hcy levels,neurological deficits,changes in activities of daily living and clinical efficacy before and after treatment were compared between the two groups.Results After treatment,the brain edema volume,SP and Hcy index levels,NIHSS score and ADL score were improved in the two groups(P<0.05),and the improvement of each index in the study group was significantly better than the control group(P<0.01);The total effective rate of the study group(90.91%)was higher than that of the control group(72.73%),the difference was statistically significant(χ^(2)=6.111,P=0.013).Conclusion Oxiracetam combined with butylphthalide in the treatment of elderly hypertensive cerebral hemorrhage can significantly improve the therapeutic effect.It can reduce cerebral edema while increasing SP level,lowering Hcy level,improving neurological deficits,and improving patients'ability of daily living.
