血液肿瘤患儿及监护人利用EQ-5D-Y量表进行生命质量评价的一致性研究

摘　　要：目的:评价血液肿瘤儿童及其监护人利用儿童及青少年版五维健康量表(EQ-5D-Y)进行生命质量评价的一致性。方法:对96名血液肿瘤患儿及其监护人进行面对面的EQ-5D-Y量表问卷访问,分别采用三水平量表(EQ-5D-Y-3L,简称3LY)和五水平量表(EQ-5D-Y-5L,简称5LY),对患儿及其监护人量表各维度、健康效用值(EQ-Index)及视觉模拟量表(EQ-VAS)的一致性进行分析评价,并分析监护人教育程度、与患儿的关系,以及患儿的病程、年龄和健康状态对一致性的影响。结果:96名血液肿瘤患儿中年龄8~10岁的占54.17%,11~17岁的占45.83%,男女比例接近2∶1。患儿与监护人EQ-Index-3LY的均值分别为(0.76±0.19),(0.77±0.21);EQ-Index-5LY的均值分别为(0.81±0.19),(0.82±0.22);EQ-VAS均值分别为(85.8±15.1),(81.2±14.1);3LY和5LY量表的EQ-Index一致性非常好(ICC>0.8),EQ-VAS的一致性一般(ICC=0.40);两个量表五个维度的一致性为中等及以上;监护人教育程度不高、父亲身份及患儿病程较短、年龄较小、健康状态不佳,都会对两个量表的相应维度和健康效用值造成一定的影响。结论:监护人可采用3LY和5LY量表代为评价血液肿瘤患儿的生命质量,但不建议使用EQ-VAS。在量表使用过程中需关注监护人的教育程度、监护人与患儿的关系,以及患儿病程、年龄及健康状态等因素的影响。Objective:To assess the agreement of EQ-5 D-Y between children with hematological malignancies and their caregivers on quality of life.Methods:A face-to-face EQ-5 D-Y questionnaire interview was conducted among 96 children with hematological malignancies and their caregivers,using EQ-5 D-Y-3 L(3 LY)and EQ-5 D-Y-5 L(SLY),respectively.The agreement of the scale dimensions,EQ-index and EQ-VAS were analyzed.The effects on agreement of caregiver’s education,relationship with children,and the child’s duration of disease,age and health status were also analyzed.Results:Among the 96 children with hematological malignancies,8-10 years were 54.17%,while 11-17 years were 45.83%.The ratio of boy and girl was close to 2∶1.The mean value of EQ-Index-3 LY of children and caregivers were(0.76±0.19)and(0.77±0.21),respectively,and the mean value of EQ-Index-5 LY were(0.81±0.19)and(0.82±0.22),respectively,the mean value of EQ-VAS were(85.8±15.1)and(81.2±14.1),respectively.The agreement of EQ-Index-3 LY and EQ-Index-5 LY were very good(ICC>0.8),while the agreement of EQ-VAS was fair(ICC=0.40).The agreement of the five dimensions of the two scales were moderate and beyond.While the caregiver’s education was not high,fatherhood and the child had a short duration of disease,was young,and the health status was poor,maybe affect the agreement.Conclusion:Caregivers can use 3 LY and 5 LY scales to evaluate the quality of life of children with hematoma,but EQ-VAS is not recommended.During the use of the scale,attention should be paid to the educational level of the guardian,the relationship between the guardian and the children,as well as the influence of such factors as the course of disease,age and health status of the children.
