多焦点人工晶状体植入术联合rhEGF滴眼液治疗白内障的疗效及对泪膜破裂时间、泪液分泌试验的影响

摘　　要：目的研究多焦点人工晶状体植入术联合重组人表皮生长因子(rhEGF)滴眼液治疗白内障的疗效及对泪膜破裂时间、泪液分泌试验的影响。方法前瞻性选取2018年3月至2020年7月简阳市人民医院眼科收治的160例(160只眼)白内障患者,随机数字表法将其分成观察组和对照组,每组各80例。对照组患者行多焦点人工晶状体植入术,观察组患者在对照组的基础上联合应用rhEGF滴眼液。比较和分析2组患者临床疗效、视功能、生活质量、泪膜破裂时间、泪液分泌试验及安全性等参数。结果治疗后1个月,2组患者裸眼远视力、矫正远视力、裸眼近视力及矫正近视力差异无统计学意义(P>0.05)。2组患者视功能评分较治疗前明显升高,且观察组患者视功能评分明显高于对照组,差异均有统计学意义(P<0.05)。2组患者生活质量各维度评分和总分较治疗前明显升高,且观察组患者生活质量各维度评分和总分明显高于对照组,差异均有统计学意义(P<0.05)。2组患者泪膜破裂时间和泪液分泌试验数值较治疗前明显升高,且观察组患者泪膜破裂时间和泪液分泌试验数值明显高于对照组,差异均有统计学意义(P<0.05)。治疗期间,2组患者均出现弥漫性表层角膜炎、睑板腺功能障碍及睑缘炎等不良反应事件,但2组差异无统计学意义(P>0.05)。结论多焦点人工晶状体植入术应用于治疗白内障可有效恢复患者远近视力,而联合rhEGF滴眼液可进一步改善视功能和生活质量,提高泪膜破裂时间和泪液分泌试验数值,安全有效,值得临床进一步研究并推广。Objective To explore the clinical efficacy of multifocal intraocular lens implantation combined with rhEGF eye drops in the treatment of cataract and its effect on tear break-up time and Schirmer test.Methods A total of 160 patients(160 eyes)with cataract admitted to the Department of Ophthalmology,Jianyang People's Hospital were prospectively selected From March 2018 to July 2020,and were randomly divided into observation group(n=80)and control group(n=80).Patients in the control group were treated with multifocal intraocular lens implantation,while patients in the observation group were treated with rhEGF eye drops on the basis of the control group.The clinical efficacy,visual function,quality of life,tear break-up time,Schirmer test and safety between the two groups were compared and analyzed.Results After treatment,there was no significant difference in uncorrected distant visual acuity,corrected distal visual acuity,uncorrected near visual acuity and corrected near visual acuity between the two groups(P>0.05).The visual function score of the two groups was significantly increased,and the visual function score of the observation group was significantly higher than that of the control group(P<0.05).The scores of all dimensions and total scores of quality of life in the two groups were significantly improved,and the scores and total scores of quality of life in the observation group were significantly higher than those in the control group(P<0.05).The tear break-up time and Schirmer test were significantly increased in the two groups,and the tear film rupture time and tear secretion test in the observation group were significantly higher than those in the control group(P<0.05).During the treatment,adverse reactions such as diffuse superficial keratitis,meibomian gland dysfunction and blepharitis occurred in both groups,but there was no significant difference between the two groups(P>0.05).Conclusion Multifocal intraocular lens implantation can effectively restore the distant and near visual acuity of patients w
